The journey begins Day 1-29 Journey Work Related Requirements Day 1) Report health condition/self certify Day 1-14: all work requirements switched off Day 8-14) Record fit notes Day 15+: work preparation and work search restart Day 14-28) Keep recording fit notes Day 29) Referral for Work Capability Assessment

The journey continues Day 29+) Referral recorded on MSRS (Medical Services Referral System), to notify Centre for Health & Disability Assessment(CHDA) CHDA sends out the health questionnaire (UC50) Claimant fills the questionnaire and return it to CHDA in the envelope provided (max 4 weeks) CHDA can request further information: GP/ f2f or phone assessment CHDA records a recommendation (fit, LCW, LCWRA) on MSRS
The journey ends Decision Maker Administrator create make a decision to do Decision Maker records the decision & inform claimant via journal message 1 month for mandatory reconsideration Possible additional award Labour Market change
