2. Rapid Rehousing is a short-term housing intervention informed by the Housing First Model. Rapid Rehousing connects families and individuals experiencing homelessness in Kern County to permanent housing through a tailored package of assistance offering supportive services and financial assistance.

3. Has a primary nighttime residence that is a public or private place not meant for human habitation; Is living in a publicly or privately operated shelter designated to provide temporary living arrangements (including congregate shelters, transitional housing, and hotels and motels paid for by charitable organizations or by federal, state and local government programs); Is exiting an institution where (s)he has resided for 90 days or less and who resided in an emergency shelter or place not meant for human habitation immediately before entering that institution Is fleeing, or is attempting to flee, domestic violence;

6. Supportive Services Supportive Services Assessment of Barriers/Resolutions Housing Navigation/ Housing Locators Financial Planning and Goal Setting At Entry After 60 days Wrap Around Services

7. Homeless families and individuals who have had an assessment done through the Coordinated Entry System are matched on a weekly basis through the prioritized housing list, depending on funding availability.

8. Program Referral All vitals including ID, Social Security Card,& Birth Certificate for children Income for all household members for the last 30-days Homeless Certification

9. CalWorks Housing Support Program (must be a CalWorks recipient) Rapid Rehousing Bringing Families Home (must be enrolled in Family Maintenance or Family Reunification through the child welfare system) Homeless Prevention (must be at-risk of homelessness; must be at or below 30% AMI)

Rapid re-housing is a model for helping homeless individuals and families obtain and maintain permanent housing, and it can be appropriate to use as a bridge to other permanent housing programs.
