  11. Signed cover letter from authorizing official 1. A one-paragraph description of the project 2. An assurance that there is sufficient financial support for the project until invoices and reimbursements have been processed (by SCDD) 3. A required statement of agreement of the terms and conditions and that the project, as proposed, will be completed

  12. Project Data Sheet A separate attachment that is included with the RFP or may be found on the SCDD website Requires proposer, project and related contact information Signed by a person in the proposal organization with the legal authority to enter into a binding contract

  14. Budget Detail Sheet This is an Excel spreadsheet that lists and calculates all project costs Details for how to complete the Budget Detail Sheet can be found in Presentation #16

  15. Continuation of Project This is an optional section of the proposal that is only included if the project will continue after the grant cycle has ended Identify the source of continuation funding Add a separate letter or provide information in the Outcomes and Evaluation section

  16. Project Organizational Chart Provide an organizational chart for the project staff alone (will also include any sub-contractors) List the names and position titles of all project staff and sub-contractors listed on the Budget Detail Sheet

  17. Curriculum Vitae/Resumes and Duties A Curriculum Vitae (CV) or resume for all personnel listed on the Budget Detail Sheet Include: A Duty Statement Relevant licenses or credentials If project staff has not yet been hired, provide a position description

  18. Previous Grants or Awards: Over the past 2 years that benefit people with I/DD Provide a list organized by: Name of Project Funding Source Contact Person Telephone Number Amount of the Grant/Award

  19. Letters of Support Applications require a minimum of 3 letters of support (from a total of 3 different entities) From any collaborators working on the project who are not funded by the proposed project One letter must be from an individual with recognized expertise related to the project

  20. Letters of Support (1 of 2) Letters of support should address: 1. Familiarity with the proposer/agency 2. Support for the project being proposed Each letter must include the agency/individual s name, address, contact name and phone number.

  21. Letters of Support (2 of 2) **Note: Letters of support may not be provided by SCDD Council members or staff, State department appointees, or entities that will receive any funding from the proposed project

  22. Proposal Checklist This is a list of all of the required attachments Must be included as an attachment

  23. Memorandum of Understanding (MOU) or Letter of Commitment (if required) Only for those projects that require a formal agreement with another entity in order to carry out the work of the project May pertain to the use of funds, facilities or access to a program, school or other entity May also pertain to coordination between agencies to achieve the aims of the proposed project

  24. Governing Board Approval (if required) Some entities have a governing board that must approve the application or accept the award If yes, please provide the proof of such approval with the application
