No action is required if: No health plan related changes AND Not enrolling in a Flexible Spending Account for 2021/2022 AND Not participating in Premium Rewards You can: Enroll in, change your health plan or waive coverage Elect or remove optional buy-ups to a plan such as expanded dental or vision Remove or add eligible family members Enroll in Flexible Spending Account Participate in Premium Rewards Human Resources

Flexible Spending Accounts Your time to enroll in a Health or Dependent Care FSA or Both Flexible Spending Account Contributions Health FSA: up to $2,750 per plan year Dependent Care FSA: up to $5,000 Minimum FSA is $10 per pay period ($240 for the year) Administrative Fee is $2.10 deducted monthly for one or both Human Resources

Flexible Spending Accounts Use It or Lose It: Current plan year: Plan year ends June 30, 2021 Must submit current year claims for reimbursement by September 30, 2021 Dependent Care ONLY: 2020-2021 plan year has been extended July 1, 2021 to October 31, 2021 for new claims & submission All claims must be in by November 30, 2021 New plan year 2021-2022: No new expenses after June 30, 2022 Have until September 30, 2022 to submit claims for reimbursement Human Resources

  11. Flexible Spending Accounts Health FSA: Use pre-tax dollars to pay for eligible health care expenses Copays, coinsurance and deductibles Other out-of-pocket eligible medical expenses Receive PayFlex Mastercard to use for these eligible health care expenses Human Resources

Flexible Spending Accounts Dependent Care FSA Use pre-tax dollars for work-related dependent care expenses Care for your child under age of 13 Care for your spouse or other qualifying dependent who is physically or mentally incapable of self-care and lives in our home more than half of the year Human Resources

Premium Rewards COVA Care and COVA HealthAware ONLY Employees and any covered dependent over 13 can complete a health assessment (requires separate login for each person covered) Employee OR Spouse completes assessment: save $17 per month ($204 per plan year) Employee AND Spouse completes assessment: save $34 per month ($408 per plan year) Human Resources

Premium Rewards To Earn A Reward BEGINNING July 1, 2021: Complete or update your health assessment between May 3 and May 17 Assessments submitted before May 3 will not count for the new plan year Refer to the step-by-step instructions included in the Spotlight Newsletter Review the Premium Rewards Requirements located on the DHRM website Human Resources

How To Enroll Or Make Changes Submit Open Enrollment Elections Online With EmployeeDirect Register if you are a new user or log in with your user name and password Online elections must be completed by 11:59 PM on May 17 Select Health Benefits Direct Update personal information if needed Enroll or make changes to health insurance plan Enroll in one or both Flexible Spending Accounts Human Resources

How To Enroll Or Make Changes Complete and submit the fillable 2021-2022 Enrollment Form The enrollment/change form was mailed to your home Available on HR website or DHRM website Print, complete, and sign and return to HR: In person: HR Office on the second floor of Tyler Place (above Moe s) Mail: Radford University Human Resources, PO Box 6889, Radford VA 24142 or Fax: 540-831-6278 or Email: Upload using SendSecure, you will receive immediate confirmation of receipt when successfully uploaded Forms must be received by 5:00 PM on Monday, May 17, 2021 Human Resources
