HWC Portal - Facility Information and Reporting Criteria
Facility-wise information and reporting criteria for Health and Wellness Centers including daily service utilization reporting, monthly service delivery reporting, HWC planning sheets, and code mapping. Criteria outlined for Sub Health Center-HWC and Primary Health Center-HWC/Urban Primary Health Center-HWC. Data entry status for Ayushman Bharat Health and Wellness Centers provided along with service delivery reporting status from May-Sept 2019. NCD screening status detailed for various states through the AB-HWC portal.

Functionality Criteria Criteria for SHC-HWC: 1. CHO in position+ 2. Training on NCD completed for MPW(F) and ASHA+ 3. Diagnostics available + 4. Medicines available+ 5. Branding completed + Screening of NCDs- Hypertension + Diabetes+ Oral Cancer + Breast Cancer started Criteria for PHC-HWC & UPHC- HWC: 1. MBBS MO in position + 2. Training on NCD completed for MO or Staff Nurse+ 3. Diagnostics available + 4. Medicines available + 5. Branding completed + 6. Screening of NCDs- Hypertension + Diabetes + Oral Cancer + Breast Cancer started Criteria for SHC-HWC: 1. CHO in position + 2.
