Auditors Term of Office one year may be extended annually, upon application maximum three years Conduct audits two maintenance programmes first quarter of the year fair, transparent and consistent manner Complete in a timely manner to meet audit completion deadlines Appreciation of service five CHSC Maintenance Points (letter from Education Director
Auditor independent auditing, double-blind no collaboration between Auditors, no collaboration between Auditors and CHSC - CSSE Office obtains requested clarification and/or additional information/documentation no discretionary points - even if it were a lack of one point to pass an audit CHSC Audit Team Chair adjudicates disagreement between audit results
Auditor take a leave of absence when due to be audited report conflicts, e.g. know the person being audited notify CHSC Audit Team Chair and CSSE Staff when unable to complete an audit provide feedback for improvements to the CHSC Audit programme
Support to Auditor CSSE Office Support obtain additional clarification, information, documentation from CHSC for Auditors provide advice or assistance in accessing maintenance programs monitor audit progress, prepare summary reports for CHSC Audit Team Chair
Audit Worksheet CHSC Maintenance enter the data from previous five years from 01.01.2011 to 31.12.2015 enter date and year in numerical format enter month as a word elaborate when needed especially when points are not awarded Result Sheet points are totalled automatically summary of your conclusion per heading
Continuous Education Courses, seminars, workshops and conferences Able to enhance the professional competence of the CHSC Evidence showing successful completion Dated transcripts, certificates, diplomas Course description Course calendar, e.g. prospectus from colleges, university, etc.
Continuous Education Attending trade show to promote product or service - no points Online training and webinars minimum three hours can be a programme delivered in segments totaling the minimum hours
Professional Designations Acquisition of Designation CRSP, CHRP, CSP, CMIOH,CIH, ROH only at this point Courses/exams involved not counted Must be earned within the CHSC s five year maintenance program period, i.e., after earning the CHSC Maintenance of Designation Courses taken to maintain other designations may be used to maintain CHSC
Professional Practices - Documentations Internal Consultants External Consultants a scanned signed letter on company or organization letterhead a declaration in writing an email to the CHSC Qualifications and Audit Committee using the company s email address a business license with founding or start- up date Description a letter of good standing with the WCB, and title & relationship with the CHSC the company s URL to the company s website/brochure, or CHSC s title & functions start date a list of clients served or billing for service provided invoice numbers number of continuous years in this responsibility CSSE may request information from the persons/organisation whose name is provided % time spent on OH&S functions
External Consultants a declaration in writing a business license with founding or start-up date a letter of good standing with the WCB or other equivalent provincial body the company s URL to the company s website/brochure a list of clients served or billing for service provided invoice numbers CSSE may send request to the companies for affirmation
Leadership and Volunteer Activity Possible leadership and volunteer activities in the safety profession (outside of Professional Practice) chairing and/or serving on association and professional organization boards of directors, committees, task forces, etc authoring or publishing (books, magazine or journal articles) presenting safety-related topics at conferences, institutes, etc mentoring such as presentations at high school/college career nights, study groups, etc.
LVA Criteria Each volunteer position = 5 points/year Publishing an OHS-related paper or article in public media = 5 points Presenting an OHS-related topic at a professional conference = 5 points Publishing an OHS-related text = 20 points Developing a university or college-level course = 20 points mentoring: 1 hour = 1 point
Leadership and Volunteer Services Service Criteria Course development/delivery a course that is developed and/or delivered for an organization other than the CHSC's employer or client Paper published Name of the Journal and the bibliography of the article, including issue volume, number, month Books authored Bibliography - publisher, ISBN, date Topic presented at a conference/seminar Official Conference brochure Testimonial Other Health and Safety Related non- committee related volunteer service Minimum eight hours By position Testimonial Mentoring exclude staff
LVA Documentation for a member of an OHS related volunteer committee or function as a volunteer (not required as part of Professional Practice) a dated and signed letter from a Chair or colleague on company or organization letterhead, or an email addressed to the Executive Director of CSSE (or designate) attesting to the CHSC's involvement and leadership/volunteerism provided;
LVA Documentation For a course developed/delivered: a dated and signed letter on company or institution letterhead, or an email addressed to the Executive Director of CSSE (or designate) attesting to the CHSC's involvement; a description of the course or subject outline and its objectives from a calendar, brochure, or website indicating target audient and delivery of classes or modules;
LVA Documentation For a presentation/seminar/workshop at a conference or symposium a dated and signed letter on institution letterhead, or an email addressed to the Executive Director of CSSE (or designate) thanking or attesting to presentation; a description of the presentation or subject outline and its objectives from a conference program, brochure, or website; institution feedback or summary evaluation form;
LVA Documentation For a published paper or book (author or editor) a dated and signed letter on publisher or institution letterhead, or an email addressed to the Executive Director of CSSE (or designate) attesting to the CHSC's authorship; a description of the paper or book from a calendar, brochure, or website indicating its authorship, target audience, availability, etc;
LVA Documentation For mentoring a colleague or relative newcomer to the OHS field outside of the CHSCs normal Professional Practice responsibilities a dated and signed letter on institution letterhead, or an email, from the CHSC's supervisor attesting to the mentorship the CHSC provided; a dated and signed letter on institution letterhead, or an email, from the individual(s) having benefited from the CHSC's mentoring attesting to the time involved
