  3. BENEFITS ELIGIBILITY Graduate staff 0.50 FTE/half-time/20 hours a week or more Graduate students with fellowships administered as assistantships The above may enroll their legal dependents with them. Check with your department s business office if you are unsure about your eligibility.

WHAT HAPPENS IF YOU DONT ENROLL OPEN ENROLLMENT IS AUGUST 1ST TO SEPTEMBER 10TH, 2018 5PM ET New graduate staff deadline: 30 days from hire or 5PM ET Sep. 10th Medical Insurance Domestic students: May not enroll in medical insurance for the 2018-2019 plan year without a qualifying life event (i.e., loss of coverage) until the next open enrollment. International students: Will have $200 late fee, hold placed on academic record until enrolled in the international student health insurance plan with the Student Insurance Office in PUSH, Rooms 338-340. Find out if you are eligible to apply for a waiver at the AHP Portal. Dental and Other Voluntary Benefits Any previously elected benefits from 2017-2018 will roll forward, but you will not be able to enroll in the two new voluntary insurance benefits (Supplemental Hospital, Universal Life).

Deductible only waived for emergency care visits to Urgent Care (and Emergency Room) when PUSH is closed. University-mandated and travel immunizations covered 100% at the Purdue University Student Health Center (PUSH). Graduate Staff Health Plan Change Deductible now waived for emergency care visits to Urgent Care (and Emergency Room) when PUSH is closed. Graduate Staff Health Plan Change Virtual acute care and behavioral health visits through LiveHealth Online (LHO) No telemedicine program New program

BENEFITS GRADUATE STAFF HEALTH PLAN In-Network Out-of-Network $200 per insured person $400 per insured person Deductible Coinsurance (you pay) 10% 30% Out-of- Pocket Maximum $1,500 per insured person $3,000 per insured person $3,000 per family* $7,000 per family* *Family means graduate staff plus one or more dependents for the purpose of the deductible and out-of- pocket maximum When the deductible applies, you must pay it before benefits begin. Coinsurance is based on the maximum allowed amount. No one person covered under a family plan will pay more than their individual out-of- pocket maximum. Once the family maximum has been met for the entire family, the plan will pay 100% of covered (i.e., not excluded) services.

VACATION AND HOLIDAYS Fiscal Year Graduate Staff (working 12 months) Accrue 2 days of vacation per month except in September, March (1 day), maximum of 22 days per fiscal year Must complete three months of service before vacation can be requested. Unused vacation forfeited upon termination or transfer to other employment classification (faculty, administrative professional, etc.) Eligible for the ten (10) official University holidays. Academic Year Graduate Staff (off in the summer) In paid vacation status during the periods of the academic year when classes are not in session (including University holidays) with the exception of the seven calendar day period prior to the 1st day of class, and the final date for submitting grade reports.

  23. SICK LEAVE EMPLOYEE Academic & Fiscal Year Graduate Staff Eligible for 2 weeks (10 working days) within a 12 month period. Illness is defined as a staff member s own illness, injury, or pregnancy.

  24. SICK LEAVE FAMILY ILLNESS Academic & Fiscal Year Graduate Staff Eligible for three (3) working days per fiscal year for immediate family illness. Spouse, parents, children, grandparents, grandchildren, sisters, brothers, and corresponding in-laws and step-relatives. Family members who reside in the employee s home are considered immediate family .

  25. FAMILY AND MEDICAL LEAVE ACT (FMLA) Academic & Fiscal Year Graduate Staff Grants eligible employees the right to take twelve (12) workweeks of job-protected leave over a twelve-month period for FMLA covered medical and family reasons. Serious health condition (employee or family member), birth, adoption, or placing a child in the employee s home for foster care. Must be employed at Purdue for 12 months, consecutive or non- consecutive, and have worked at least 1,250 hours in the 12 months preceding the date leave/time off commences. Eligibility must be met prior to the birth/adoption

  26. PAID PARENTAL LEAVE Academic & Fiscal Year Graduate Staff Must be employed at least 12 continuous months in a half-time (20 hours a week) or more position Eligible for up to 240 hours of paid leave during first 12 months following birth or adoption

  27. BEREAVEMENT LEAVE Paid Bereavement Leave Days Relation Immediate relative Spouse, parent, child, grandparent, grandchild, sibling, corresponding in-law or step-relative Up to 5 days over six consecutive calendar months Academic & Fiscal Year Graduate Staff Relative living in employee s home Uncle, aunt, niece nephew, or first cousin if the relative lived in the employee s home Up to 5 days over six consecutive calendar months Relative Uncle, aunt, niece, nephew, or first cousin 1 day Fellow Employee - Subject to staffing needs of employee s department as determined by head of department 1 day

  28. JURY AND WITNESS DUTY Academic & Fiscal Year Graduate Staff Graduate staff are eligible for paid leave as a juror or court witness as a result of a subpoena issued by a court. Service as a volunteer expert witness or other volunteer court duty is not included.

  29. LEAVE REQUESTS See your business office to complete any forms associated with a leave of absence Leaves greater than 22 consecutive work days for any reason other than vacation or FMLA must have the approval of the Dean of the Graduate School.   30. RISK MANAGEMENT PROGRAMS Academic and Fiscal Year Staff Worker s Compensation Covers injuries incurred while performing duties as an employee of the University (not in student capacity) and while traveling on University business. Graduate Staff Employment Manual

  31. REMINDERS Returning graduate staff must enroll by September 10th 5PM ET New graduate staff must enroll by September 10th 5PM ET OR 30 days from their date of hire, whichever is later. The same deadline applies for those enrolling in the domestic or international student health insurance plans (for those intending to cover dependents and wish to take one of these plans instead). International Graduate Staff must complete their immunization requirement by September 30th, 2018.
