This template serves the goal of standardizing Aquifer case debriefs, including reviewing case objectives, chief complaint, initial differentials, history, physical exam components, pertinent schemas, medical knowledge review, clinical skills, clinical reasoning, differentials rationale, and management concepts. It ensures a structured approach while allowing flexibility for unique cases.

  1. Case Debrief - TEMPLATE Case#/Case Name:

  2. Instructions for Use This template has been developed to serve the goal of standardization of Aquifer case debriefs. This template may also be used to standardize case debriefs using the PPOR or any other case format. Having said that, it is understood that each case is unique, community specialists may conduct the debrief, and therefore not all of the components provided here may make sense for each case. (i.e. community specialists may not understand the schemas). Please choose your own adventure include those slides that are helpful and disregard those that may not be.

  3. Case Objectives (Found on Aquifer Case Summary in each case or within the Block/thread Objectives document for that week)

  4. Review of Case Items to be included in this slide/section: Chief complaint Discussion of initial ddx based on CC Review of the main components of the history and physical exam presented in the case

  5. Schemas Review of the pertinent schemas

  6. Medical Knowledge Review Choose one or more medical knowledge topics from the case to review. This could be a basic science topic, pathophysiology concept or clinical presentation review i.e. discussion of the pathophysiology of jaundice. This discussion may be appropriate in other places in the debrief as well - Ex: An in-depth knowledge discussion of asthma treatment options would fit better after the Management portion of the debrief.

  7. Clinical Skills Use this section to review clinical skills highlighted in the case (i.e. Abdominal exam special tests, review of components of the pertinent history or review of targeted history items such as taking a sexual history, screening for alcohol misuse, etc.)

  8. Clinical Reasoning Activities in this section can include working through the differential, creating an illness script, etc. Top Differentials Rationale

  9. Management This section can be used to review the concepts behind labs, diagnostic imaging or treatments (lifestyle interventions, medications, referrals, patient education, etc.) pertinent to the case

  10. Appropriate Next Steps This section can be used to review timing for follow-up visit, return precautions, etc

  11. Visit Management Discussion of billing, coding, etc.
