Updated CDC Recommendations NEW CDC RECOMMENDATION: TST or IGRA tests for TB can be done before, after, or during the same encounter as a COVID-19 vaccination COVID vaccination should not be delayed because of TB testing Previous recommendation: Wait at least four weeks after COVID-19 vaccination to perform a TST or IGRA Recommendation has been updated because of issues with delaying TB testing
Trial of Four-Month TB Treatment Regimens Published May 6, 2021, New England Journal of Medicine. DOI: 10.1056/NEJMoa2033400. Big Picture: Would a 4-month treatment regimen using Rifapentine and/or Moxifloxacin give outcomes *at least as good* as the current standard 6-month regimen? Noninferiority Trial. Noninferior = *not worse than* the standard or control regimen. All data and figures are from the NEJM article and/or accompanying Research Summary, or from a webinar as noted. * = Peter s own words.

Efficacy Results Percent of population with TB disease. Cut-point for Noninferiority: Left is better. Rifapentine confidence interval crosses cut-point: Rifapentine was Inferior to Control.
Safety Results Incidence of adverse events similar between Rifapentine- Moxifloxacin and Control.
*Summary* 4-month treatment containing Rifapentine & Moxifloxacin was Noninferior compared to standard 6-month regimen. 4-month containing Rifapentine was Inferior. Remember this was a Trial, not Recommendations. No recommendations for or against using the Rifapentine-Moxifloxacin regimen. Considerations. Rapid drug-susceptibility testing for: Fluoroquinolones, Isoniazid, Rifampin. E.g., CDC s Molecular Detection of Drug Resistance (MDDR). Rifapentine is absorbed better with high-fat foods. Rifapentine/Moxifloxacin regimen may cost more per-dose than standard regimen. Will fewer total doses offset higher per-dose cost?
