Annual Team Assessments (14+) Scheduled annually to include: parent, district representative, Service & Support Administrator, Therapists, School Administrators, Outside Agencies, Teacher, and any other person/agency who works with the student. 60 Minutes Maximum Agenda (see handout/outline) Review of recent assessment(s) (see list) Review of upcoming IEP goals (leave w/an idea of next steps) Leave with an idea of where to go with transition plan and educational activities in IEP
