COVID-19 Vaccination Eligibility All Michigan residents age 16 and older 50 to 64 years of age 16-49 years of age with disabilities or pre-existing conditions 65 years of age and older 50-64 years of age with disabilities/medical conditions Healthcare workers Long term care residents/staff Prioritized frontline responders School and childcare staff Mortuary service workers Corrections staff and detained individuals Congregate care facilities Agriculture/food processing workers Essential frontline workers Caregivers of children with special health care needs Currently Eligible Beginning April 5 Beginning March 22 2 2
Ford Field Community Vaccination Site 6,000 doses a day: 5,000 on-site and 1,000 distributed through mobile efforts working with 4 local health departments. Vaccinations begin Wednesday, March 24, and will operate from 8 a.m. to 8:30 p.m., seven days a week, for eight weeks. Partnership between State of Michigan, Federal Emergency Management Agency (FEMA), Wayne County, City of Detroit, Ford Field, Meijer, Henry Ford Health System, & the Detroit Lions. Vaccine doses administered at the Ford Field Community Vaccination Site will target Michigan s most vulnerable populations using the Social Vulnerability Index (SVI). Vaccine doses at Ford Field will be in addition to Michigan's regular statewide vaccine allotment. 3 3
Ford Field Registration Registration and scheduling for vaccine appointments is done through Meijer. Registration is now LIVE! Online at https://clinic.meijer.com/register/CL2021 By texting EndCOVID to 75049 Individuals that do not have access to the internet, or who need assistance can also call the COVID-19 hotline at 888-535-6136 and press 1. Note that the call center will have long wait times and people should only call if they cannot use the web option. Registered individuals will receive a text message or phone call when it is their turn to get scheduled for a vaccination and will be provided instructions on how to schedule their vaccine appointment. 4 4
Accessibility & Supportive Services at Ford Field Accessibility for Individuals with Disabilities Accommodations on site will be available and can be requested by completing the Accessibility and Transportation Request format the time of scheduling. Transportation Transportation to the Ford Field Community Vaccination Site will be available free of charge for residents of Macomb, Oakland, and Wayne Counties and the City of Detroit, when needed. To request transportation, individuals should complete the Accessibility and Transportation Request format the time of scheduling. Requesting Supportive Services A link to the Accessibility and Transportation Request form will be provided in the confirmation email sent to the individual after an appointment is scheduled. For individuals using the COVID Hotline for scheduling support, the call agent will assist in the completion of the form over the phone. 5 5
