Achievements and Challenges in Improving AKI Management at NHS Fife

What is your biggest achievement in the collaborative? In hospital: Getting the AKI alerts in place quickly and provide linked guidance. Knowing where the high AKI rates are. In the community: AKI alerting to GPs has increased awareness of the condition; our GP survey showed that 53% of alerts changed management; 20% of alerts changed patient outcome , a further 30% may have changed outcome .
What was key to your success? Enthusiastic engagement and hard work from Heather Holmes and her biochemistry staff Involvement of renal consultant Arthur Doyle Communication to the wider medical and nursing community about AKI via Grand Rounds, posters and teaching sessions.
What challenges have you overcome? IT challenges - working across different systems Getting SHOUT AKI message across to as many staff as possible
What is next for AKI in your board? Getting the AKI alerts on to our Patientrack system will ensure that medical staff have to go through the AKI guidance whenever they get an alert, and it will also be visible to all staff dealing with that patient. Investigation of AKI 2 and 3 cases to look for common factors Increased use of Sick Day Rules cards AKI in orthopaedics a big issue to be investigated Can we reduce AKI in hospital?
