  3. Why focus on outreach? Very large unsheltered homeless population (70% of all homeless 2019 PIT) Proactive, flexible outreach not restricted by geography or subpopulation is an essential element of an effective crisis response system Built for Zero scorecard assessment and local data point to areas of focus for targeted improvement County & City of Sacramento and Cities of Citrus Heights, Elk Grove, Folsom, and Rancho Cordova have individually prioritized outreach through funding Sacramento has new outreach guidelines to pilot Outreach provider interest in coordination and collaboration is high

Analysis of Outreach Inventory Sacramento County has 14 programs performing an outreach type function in our community which provides 103 outreach staff In reach vs Outreach: 1 program performs in reach Representing 7% of programs and 22% of total outreach staff 13 programs perform outreach Representing 93% of programs and 78% of total outreach staff Countywide vs geographically limited: 5 programs are not limited by specific geographic area Representing 36% of programs and 72% of total outreach staff 9 programs are limited by a specific geographic area Representing 64% of programs and 28% of total outreach staff Enters data into HMIS: 7 programs fully enter data into HMIS Representing 50% of programs and 17% of total outreach staff 6 programs do not enter into HMIS Representing 50% of programs and 83% of total outreach staff Referral/responsive vs proactive: 5 programs perform outreach by referral or response Representing 36% of programs and 59% of total outreach staff 9 programs perform proactive outreach Representing 64% of programs and 41% of total outreach staff

Outreach Guidelines- Overview Developed with technical assistance from Technical Assistance Collaborative (TAC), funded by CA HCD and provided to the County of Sacramento Collaborative development process involving many stakeholders Purpose: Establish guidelines on how street outreach resources and interventions function within the crisis response system and work towards more consistent practices across programs Audience: Providers, people experiencing street/unsheltered homelessness, system funders, system planners 2021 Sacramento Continuum of Care Gaps Analysis

Outreach Guidelines- Developing Outcome Measures Outcomes we should measure: clients on a housing pathway clients offered problem-solving conversations clients moved to a permanent housing destination clients entered into HMIS clients with documents uploaded in HMIS clients linked to Coordinated Entry System These outcome measures represent a subset of all outcome measures to be included in the HMIS Reporting Template 2021 Sacramento Continuum of Care Gaps Analysis

HMIS Reporting Template For the County pilot that led to increased investment in outreach, a reporting template to capture detailed information about services provided and outcomes achieved has been developed This custom report will be built in the HMIS and can be utilized for project- level analysis by all outreach programs and their funders, as well as for system-level analysis Input on the report template will be sought from the other funders of outreach, including local government 2021 Sacramento Continuum of Care Gaps Analysis

New Outreach Collaborative Both the Built for Zero work and the new outreach guidelines call for increased coordination and collaboration across outreach teams operating in Sacramento The purpose of the new outreach collaborative is to support outreach teams through training and problem-solving opportunities, provide a forum for sharing resources, and drive toward increased consistency in implementation of the outreach guidelines and other best practices to reduce unsheltered homelessness SSF and Community Solutions have convened two planning meetings with outreach program managers/directors to date to understand their needs and interests around collaboration to secure buy-in to participate 2021 Sacramento Continuum of Care Gaps Analysis
