Definitions Workplace violence - Any act of violence or threat of violence that occurs in a place of employment. Workplace violence includes, but is not limited to, the following: The threat or use of physical force against an employee that results in, or has a high likelihood of resulting in, injury, psychological trauma, or stress, regardless of whether the employee sustains an injury. An incident involving a threat or use of a firearm or other dangerous weapon, including the use of common objects as weapons, regardless of whether the employee sustains an injury. Workplace violence does not include lawful acts of self-defense or defense of others.
Definitions Four types of workplace violence: Type 1 violence - Workplace violence committed by a person who has no legitimate business at the worksite; includes violent acts by anyone who enters the workplace or approaches employees with the intent to commit a crime Type 2 violence - Workplace violence directed at employees by customers, clients, patients, students, inmates, or visitors Type 3 violence - Workplace violence against an employee by a present or former employee, supervisor, or manager Type 4 violence - Workplace violence committed in the workplace by a person who does not work at the worksite but has, or is known to have had, a personal relationship with an employee

Procedures to Identify, Evaluate, and Correct WV Hazards An inspection of each site to identify, evaluate, and correct any WV hazard completed prior to implementation of the WVPP After each WV incident [SPED issue] Periodic inspections of each site Whenever [District] is made aware of new or unrecognized hazard
Post-incident Response and Investigation [Insert who/how a WV incident will be investigated and reported by/to reporting party] SPED related incident - if student has a 504 plan or Individual Education Program ( IEP ), remediation of any future violence may be addressed by a referral to the 504 or IEP team
Review and Revision of the WVPP [Insert procedures to gain input from staff and unions on review and revision of the WVPP] A review (and revision if needed) must be done: At least annually When a hazard is observed or becomes apparent After a WV incident
Training Must be conducted by a person knowledgeable about the WVPP Allow for Q&A When WVPP is established Conducted annually When a new or unrecognized hazard has been identified Training may be limited to changes in the plan
Recordkeeping Create and maintain records of WV hazard identification, evaluation, and correction, for a minimum of five years Create and maintain training records for a minimum of one year and include the following: o Training dates o Contents or a summary of the training sessions o Names and qualifications of those conducting the training o Names and job titles of all attending the training sessions Maintain violent incident logs for minimum of five years Maintain records of WV incident investigations for a minimum of five years o The records shall not contain medical information pursuant to subdivision (j) of section 56.05 of the Civil Code
