  1. Step 1 Learning Disability Supported Living request or Residential request comes through to the LD inbox Before we can start looking we need an email from Verifications with funding in principal or full funding for the service requested.

  2. Step 2 Saved under: O:\Socserv\WG_CONT\Adult Commissioning\Brokerage\LD Commissioning\Work Log Enter clients details Remember to mark work level colour

  3. Step 3 and Step 4 Speak to care manager allocated on Eclipse, obtain the Care Act assessment and support plan from Eclipse and save it under the service users file (File can be found under O:\Socserv\WG_CONT\Adult Commissioning\Brokerage\LD Commissioning\Service Users Folders) Files are saved alphabetically by surname (initials and swift ID). Discuss current vacancy's with the care manager need to look into what needs the SU has and who are already in the homes and if they would be suitable. You should have your availability list of where has vacancies talk to Elaine if you need help or guidance

  4. Step 5 Using an encrypted email - email the assessment over to the selected providers as agreed with the care manager Normal email format below Hi .. Please see attached assessment for AA 12345 If in principal you feel that you could meet his/her needs please let me know The request is for .. We were thinking of Kind regards .. Attach assessment and send to the provider Save sent email in the SU s folder and make a note of the work log Mark work level orange

  5. Step 6 Save all responses from the provider in the SU s folder Forward the providers email onto the allocated care manager and see if they are happy for a full assessment to take place If the care manager is happy ask for them to send over the SU s Key information sheet Save returned email to SU folder in Ldcomms folder used in previous steps

  6. Step 7 Once you receive the Key Information sheet from the care manager, save the document in the SU s folder. Once saved email it to the interested provider asking them to contact the family and arrange a full assessment Call the provider if needed to confirm this is what you are doing

  7. Step 8 and Step 9 Confirm/receive costs from provider Send this onto the care manger to be discussed on panel (LD panel is every Wednesday) Panel may ask for reasoning behind the costs if they do ask the provider to provide us with a full breakdown in the costs /Support plan this will help with the process of getting the funding agreed Once funding has been agreed and received via email from panel, notify the provider via email confirming that the breakdown has been agreed Arrange a start date or arrange any transitions work if needed prior to the move keep the care manager posted

  8. Step 10 Set the service up on Swift once the SU has officially moved into the property The social worker/LD Admin will change the address on swift Complete perm care set up form Talk to ASC finance Lee Newbury for this form Scan and send via printer to you Save copy under SU folder With the actual copy pass it on to ASC Finance (Lee or Beth) to Authorise and put on the payment run/CC etc. Update work log and move from current into completed add the start date in the end column and save
