Development of a comprehensive guidebook for supported living services aimed at individuals with I/DD, their families, providers, and community stakeholders. The initiative focuses on identifying and addressing systemic barriers hindering the utilization of supported living services, with goals to provide recommendations for systemic changes. Accomplishments to date include stakeholder identification, informative conversations, and collaboration efforts with various stakeholders. Current and upcoming activities involve regional meetings, web design, guidebook content review, and continuous engagement with stakeholders.

DEVELOPMENT OF A SUPPORTED LIVING GUIDEBOOK FOR PEOPLE WITH I/DD AND THEIR FAMILIES, PROVIDERS, MCOS, AND COMMUNITY STAKEHOLDERS TO USE AS A RESOURCE FOR IMPLEMENTATION AND OPERATIONALIZING THE SERVICE WITH BEST-PRACTICE MODELS. ENGAGE A SUPPORTED LIVING LEARNING COMMUNITY TO SHARE INFORMATION, RESOURCES, BEST-PRACTICES, AND UPDATES AS THE SERVICE EVOLVES AND GROWS STATE-WIDE. PUBLISH WRITTEN AND VIRTUALLY INTERACTIVE GUIDEBOOK MATERIALS EASILY ACCESSIBLE TO INDIVIDUALS WITH I/DD, THEIR FAMILIES, AND SUPPORT TEAMS. Initiative Goals

Participation in SL 2 & 3 Stakeholders Meetings, Action Team, and Steering Committee Collaboration with NC Medicaid Consultant to publish a SL user survey to inform enhancement of service delivery Additional Activities to date Initial engagement with website developer Identification of guidebook content topics and themes

  6. Current and Upcoming Activities Individual meetings with people using SL services from each region of NC being scheduled Web design Outline of guidebook content Ongoing engagement with SL Providers and Family Advocates to develop, review, and provide feedback to guidebook content and web design Continued monthly meetings with NCCDD Review Committee, SL 2 & 3 Stakeholders Group, Action Team, and Steering Committee
