This Quality Improvement Project aimed to enhance the efficiency and safety of the inpatient consultation service at St. Luke's Radiation Oncology in Beaumont Hospital. By recording consults in ARIA for the first time and implementing a streamlined handover system, the project successfully tracked workloads, improved team communication, and facilitated service planning. Results showed significant impact, with a total of 91 consults received and positive feedback from stakeholders on the new pathway's ease of use and risk reduction in clinical handover.

Methods: All stakeholders in the inpatient consultation process were identified and consulted .The task appears on the Radiation oncologist s task pad to update team when a new consult has been received. We have chosen appropriate headings to ensure all aspects of consult are covered prior to completion This Quality improvement project was submitted via QART and approved by the ARIA HCR group. The project began in April 2021. The QIP was run as plan, do, and study act (PDSA) cycles. After initial roll out the QIP was evaluated 2months later by way of user feedback. At this time we amended the titles of encounters with the feedback. We will continue to do 2monthly (2nd PDSA cycle) of encounters project and upgrade the service appropriately. The number of inpatient consults is logged as a metric and included in the QIP final report. Introduction: The radiation oncology inpatient consult service in Beaumont hospital is a busy and time pressurised service. Prior to this project, the number of patients were not being tracked and there was no record of workload or staff resources used. Furthermore, the St Luke s building uses ARIA online records only, unlike the main Beaumont hospital which is a paper chart system, creating significant challenges for handover between teams and / or team members. The ability to monitor progress of complex cases and ensure all team members involved were up to date was unreliable. The aim behind this project was to record consults in the ARIA for first time ever and also create a safer handover system to follow-up consults and finalise treatment plans.
Results: Conclusions: This Quality improvement project has provided a more efficient and safer consult system in St Luke s Beaumont. The workload is now recorded and the handover between teams is now completed in a precise user friendly way. The data provided will facilitate service planning going forward. (Apr-Sept2021). 13 cases were radical 45 cases were palliative 33 cases did not receive any radiation. The most common dosing in palliative cases was 8gy/1fr (n=14). Palliative RT to bone was the most common site of radiation treatment. A total of 91 consults were received in a 5 month period 25 Futures steps: This process could also be applied to phone call referrals this workload is not currently captured Site of radiation 20 Feedback to date on surveys: All stakeholders involved in the process found the new pathway easy to use Feedback suggests the risks associated with clinical handover are much reduced by this process.
