Portsmouth Provider Partnership (P3) Funding Proposal Processes P3 unites the people and organisations contributing to health and care in Portsmouth. Together, we support the wellbeing, care and health needs of our city's residents. We share experiences, collaborate, and innovate, highlighting our strengths and building on them for the benefit of our communities and neighbourhoods. The partners will develop and implement an agreed programme of change, in line with the principles of the Portsmouth Health and Wellbeing Strategy 2022-30 , which will: Improve access to services; Provide more integrated services outside of hospital Achieve efficiencies across the Portsmouth system Where appropriate/available provide suitable resources (financial and people) or support to complete necessary external funding bids to ensure the agreed programme can be delivered. Create strength through partnership to support the development of health and care services for the benefit of the patients, public and staff of Portsmouth City.
Portsmouth Provider Partnership (P3) Funding Proposal Processes Criteria to apply for resources/funding via P3. The proposal must be completed in full, with clear information and be signed off by all parties involved. Assessment criteria The proposal must demonstrate: Extent of change deliverability and sustainability of the project and proposals projected period, value and ongoing sustainability of the project beyond the duration of pilot investment outline of co-creation with users and clinicians demonstrate user engagement throughout the pilot/project and how feedback will be used to shape future developments include pilots where applicable. Strategic alignment clear demonstration of how the project aligns to the HCP 5 pillars clear evidence of the project s fit and alignment to the P3 ambitions
Portsmouth Provider Partnership (P3) Funding Proposal Processes Benefits realisation demonstrate clear aims, objectives and measurable outcomes/KPI s for both the patient/user and the system clear governance and reporting structure demonstrate a clear plan to move to business as usual/exit plan clearly identified interdependencies with related solutions clear project plan with key milestones for delivery robust project leadership and governance structure, including risk mitigation plan evidence of due diligence and assurance processes being implemented at local level the proposal must confirm that all parties involved with the project are content that the outputs will be delivered without detriment to existing good quality systems already in place.
Portsmouth Provider Partnership (P3) Funding Proposal Processes Financial viability demonstration of value for money only request funding that is additional, over and above current funding and solely attributable to the pilot demonstrate no contribution to organisational overheads / corporate recharges have considered all options to eliminate/minimise equipment costs. If equipment is purchased, should be clear as to the expected ownership, who takes on responsibility for maintenance during the pilot and at the end of the pilot what happens to the equipment (including consideration as to whether P3 should be reimbursed for equipment costs) Clinical leadership (as appropriate) executive/director buy-in and clinician engagement plan evidence of established clinical leadership structure and project expertise clear case of the project s benefit to the clinical workforce and their involvement in the project leadership

Funding over 1000. Present proposal to P3 board for agreement Funding under 1000 delegated authority for sign off via Delivery Group Funding/resources declined Funding/resources agreed Review of proposal and return to P3 board for sign off Funding issued and monitored via Solent financial processes Project monitored monthly via P3 board and delivery group Project BAU/ ended, funding Complete
