  9. Summary of Services (SOS) Log

  10. Question # 1 Question: I have all the CAA components completed. I mailed the caregiver letter, e-mailed CSW intro letter, placed staff letter in guidance counselor mailbox, and spoke with foster youth. I have not received a follow-up e-mail, phone call, or visit from stakeholders. Can I count the CAA completed? Answer: No, CAA can not be counted completed. The following steps can be attempted to meet minimum contact requirements: Follow due diligence: 3 total attempts must be made to communicate with a CSW E-mail and/or make a phone call to caregiver, CSW and guidance counselor and request a meeting. Leave LAUSD cell number and e-mail to be reached at.

  11. Question # 2 Question: An elementary student has an 85% attendance rate. I have all the CAA components completed. Parent and teacher were both at SST. However, I have been unable to meet with the student due to his absence from school. I have summoned the student three times and each time he was absent. Can I count the CAA as completed? Answer: No, this student has an identified risk factor, therefore a student contact is required. A suggested next step may be to conduct a home visit.

  12. Question # 3 Question: Is LAUSD stakeholders contact only limited to teacher and guidance counselor? Can I count other certificated/classified LAUSD stakeholders as a contact if I am unable to meet with a teacher or guidance counselor? Answer: Yes, you may communication with other LAUSD stakeholders such as PSWs, other PSA counselors, Deans and Administrators, for example.

  13. Question #5 Question: Does a questionnaire given to a specific student s teacher count as communication with the teacher? Answer: Yes, if the teacher completed the form and returned it to you.

  14. Question #6 Question: Does a monthly e-mail update to school staff (counselors, AP, Principal) providing information on the school s foster youth count as valid communication? Answer: No.

  15. Question #7 Question: I have completed a CAA. However, the court case closed (and student is still attending your school) and I never met with the student or spoke to the caregiver. Do I need to go back and meet with the student and caregiver? Answer: The yes needs to be changed to in-progress on the SOS log. In order to count as completed you need to make contact with the student and caregiver and make referrals for any at-risk factors and/or concerns. *September 19th DataMatch students must meet CAA Guidelines for Completion.

  16. Question #8 Question: I have completed a CAA. However, the case closed and I never met with the student this school year. However, I did meet with the student last year. Does this count as a student contact? Answer: No, CAAs must be completed annually and therefore, the contact requirement must be met on a yearly basis as well.

  17. Question #9 Question: Spoke to CSW but they informed me that the student has a voluntary case. I completed the CAA, but am I still expected to reach out to teacher/counselor, parents, and meet with student? Answer: No. On SOS log change from Yes to In-Progress . Communicate with FamilySource PSA Counselor and transfer CAA files.

  18. Question #10 Question: I spoke to the CSW and the parent. Parent declined services and doesn't want me to meet with student. I completed the CAA. Am I now required to meet with student 1:1 even though parent said they didn't want me to? Answer: No. Change to In-Progress . This is a student that you should continue to monitor by consulting with school staff and reviewing MiSiS (and other databases). Inform CSW that caregiver has requested that FYAPC not work with the student.

  19. Question #11 Question: Do I have to complete a CAA for a student that was on the DataMatch, but moved out of district? Do I have to communicate with the student and stakeholders? Answer: No, unless student appears on the September 19th DataMatch. A student contact is not required. However, FYAPC is to verify school enrollment and assisting with records transfer (as needed) by communicating with community and LAUSD stakeholders.

  20. Pacing Plan Complete a minimum of 1 CAA daily The program goal is to have 60 CAAs completed by each counselor by December 18, 2015 The program goal is to have 100% completed by each counselor by February 26, 2016
