The online Jackson Active Living Plan Survey can be found here: https://www.surveymonkey.com/r/CTGPPMB

The original active living plan was created in June 2015 with funding from Community Wellness Partners/SHIP and completed by SW Regional Development Commission.

It is recommended that active living plans be reviewed every 3 years on goal/strategy completion and updated every 5-10 years.

There were 140 respondents that took the survey; Please note that a number of respondents didn’t answer residency and not all respondents answered each question.

Each question had a number of options to select, with two to four options requests.
