  19. What happens if my application is not approved for a 5-year Extension? 15 The institution will be granted a provisional extension with areas the institution should address The institution will have six months six months to address the recommendations and submit a revised application

  20. What happens if my institution does not submit a 5- Year Extension Application? 16 The institution s membership in the IEP Program lapses Institution will be moved to inactive status inactive status

What happens if my institution does not submit a 5-Year Extension Application nor a 5-year Extension with a new G&I Plan within 12 months and does not continue engaging in IEP? 17 The institution s membership in the IEP Program lapses Institution will be moved to inactive status inactive status
