12 Is there a strict deadline for application submission? Yes Yes Applications are due according to the date assigned in the IEP Community of Practice End of day, 8:00 PM ET/5:00 PM PT

What happens if my application is not approved for a 5-year Extension? 15 The institution will be granted a provisional extension with areas the institution should address The institution will have six months six months to address the recommendations and submit a revised application
What happens if my institution does not submit a 5- Year Extension Application? 16 The institution s membership in the IEP Program lapses Institution will be moved to inactive status inactive status
What happens if my institution does not submit a 5-Year Extension Application nor a 5-year Extension with a new G&I Plan within 12 months and does not continue engaging in IEP? 17 The institution s membership in the IEP Program lapses Institution will be moved to inactive status inactive status
How can I reinstate my institutions IEP membership if our IEP designation program membership lapses? 18 The institution will enter the next available IEP Startup Cohort and submit a new IEP Designation Application Information regarding the IEP Designation process can be located here
19 Can an institution submit more than one application? No No An institution can only submit one application every 5-Year Extension cycle Unless notified other wise, the 5-Year Extension Application deadline is the first Friday of December
Is it true only one person per institution can edit and submit an application? 20 An institution should designate a lead to create a Momentive account and application Lead can add collaborators in Momentive and control view and edit privileges
