FAU Claims FAU Claims Workers Compensation Workers Compensation While IW was performing a procedure with a colleague, when placing a groin central line his colleague was holding the blade and when IW turn around to face him the scalpel struck the IW s LT middle finger. IW reports a laceration to his LT middle finger. While IW was conducting a fire drill, he was walking out the front door and a wasp stung him in the RT ear. IW reports a small puncture, redness, and slight swelling to his RT ear. IW not seeking treatment. While IW was working in the agriculture park with the fish, she was bit on her LT thigh. IW reports a puncture with severe swelling about 3 inches in diameter to her LT thigh. IW is not certain what bit her, possibly a black widow spider. While IW was in a surgery, the scrub tech touched exposed muscle, causing blood to squirt onto the IW's forehead and dripped into her LT eye. IW reports possible exposure. FAU Claims FAU Claims Workers Compensation Workers Compensation While IW was treating a patient in the ER Department he was splashed in the face with the patient's blood. The patient is HIV+. IW was treated at the ER. While IW was placing materials into the autoclave machine, the tubing has chemical agents, the materials had been pre sprayed with a cleaning solution agent, during the sterilization process there was a chemical reaction, he opened the machine door, and While IW was dressing a residence laceration, blood squirted from the wound into her RT eye. She washed her eyes out and went down to the ER for treatment. While IW was working between 10am and noon IW was working with organic solvents and the IW believes the prolonged exposure to the solvents created an irritation to his LT hand and fingers, which is the IW's dominate hand, IW also reports blisters. FAU Claims FAU Claims Workers Compensation Workers Compensation IW called to report while he was attempting to catch a meeting in the Performance Building, he tripped and fell on the sidewalk injuring his face. IW reports laceration with bleeding, bruising, and swelling to his nose and cheeks area. IW lost some teeth IW reports that she was leaving the building at 7:00 p.m. IW reports some of the filling was missing from the sidewalk, causing IW to trip. IW sustained a split in her upper lip, hematomas on the palms of her LT and RT hands, and scrape to the right kne While IW was walking from the office into the lobby, she slipped on the tile flooring, causing her to fall landing on her LT hip and LF elbow and striking her head on the door frame. IW reports pain to her LT hip and LT elbow. IW went for tx on 8/2/2022. While IW was exiting a building and heading to the tutoring room, she tripped on a mat and fell to the ground landing on her RT knee and LT knee. IW then braced herself with RT hand and LT hand, which are swollen and bruised. IW reports bruising and swel IW called to report that while walking in her office with things in her hands, she tripped on her feet and fell to the ground her both knees and landed on RT shoulder. IW is experiencing pain to RT knee. IW is also experiencing popping and cracking sound While IW was walking into the police dept, she lost her balance and fell to the ground landing on her LT knee. IW reports pain to her LT knee and LT ankle, and an abrasion to her LT knee. FAU Claims FAU Claims Workers Compensation Workers Compensation While IW was working with a shark trying to take a hook out of a sharks mouth the IW put her hand inside the shark s mouth to try and remove hook. While doing so IW cut the back of her LT hand/index and middle finger on the sharks teeth. IW sought medic While IW was placing a central line in a patient, she stuck her LT hand with a scalpel. IW reports a puncture wound to her LT hand. IW had her blood work done at on site medical. While IW was suturing a laceration on a patient, the patient jerked his hand back causing the IW to stick her LT index finger. IW has on-site treatment at the St Mary's Hospital ER dept. While IW was placing a chest tube with a needle, the needle slipped and stuck his LT middle finger. IW reports a puncture wound to his LT middle finger. IW had blood work drawn at Boca Raton Regional IW called to report while he was assisting a surgeon with a Patient, he was sustained a needle stick to his LT second digit finger. IW reports puncture wound with slight bleeding to his LT second digit finger. Supervisor not available. IW called to report while he was suturing a wound, when he stepped back the suture got stuck causing a puncture to the IW s LT thumb with the suture needle. IW reports a puncture wound with slight bleeding in the area. IW went to the ER to be checked out While the IW was suturing a toe laceration and while suturing he stuck himself with a needle in his RT thumb. Iw was seen by on site medical. While IW was suturing a wound , he accidently punctured his LT ring finger causing it to bleed. IW sought medical attention at the hospital he was at.
