Data from PharmaOutcomes shows NWAFT made 23 referrals for heart failure in January 2023. Of these, 17 referrals were acknowledged by community pharmacies, 2 were rejected, and 4 were not picked up. The journey included Stage 1 Discharge Referral, Stage 2 First Prescription, and Stage 3 Patient Understanding Check. Issues identified included new medicine initiation, wrong doses, and missing medications. The analysis also discusses the completion rates of each stage by community pharmacies.
