The Application Chapter Secretary or Commander will submit application packets online to the Texas State Rep. The SR can submit on behalf of the Chapter if a special temporary need occurs. Applications must contain: Chapter Designation Sponsor Information for Auxiliary or Supporter members Former member number for returning members ALL signatures and initials must be HAND WRITTEN IN INK No script fonts or e-signatures will be accepted The application and patch agreements are legal documents and must be of the current revision and must be legible. There is NO reinstatement process. Anyone removed from the CVMA roster for any reason must submit a new application with supporting documentation. A new member number will be assigned. Initial dues are paid by the Chapter Secretary or Commander via PayPal to the NBOD after the application is approved by NBOD 4 20 May 2020 REVISION 1.5
The Application The Chapter Secretary or Commander will: Complete online applications Upload all documentation. Ensure ALL documentation is legible and portrait oriented Application Packets Full Members Application DD214 Service-2 or member-4 copy, ERB, ORB, SURF, etc Must show discharge information Patch Agreement (Pulled from Repository and filled out completely) General Discharge Addendum AFTER Receiving Member #: Initial dues of $20 submitted via PayPal to the NBOD Resubmit PA with Member # to State Quartermaster (SQM) Submit patch usage fee ($50 for 12.5 patch or $40 for 10 Patch) to State Quartermaster (SQM) at txcvma@gmail.com 5 20 May 2020 REVISION 1.5

Life Members Application Patch Agreements Submit a copy of PA on file or a new PA Excel or PDF document showing the three events qualifying the member for Life Membership After the application is approved by NSEC submit dues via PayPal to the NBOD Dues of $200 for Full Member or $100 for Auxiliary/Supporter submitted via PayPal to the NBOD 7 20 May 2020 REVISION 1.5
PayPal Details The State PayPal account is txcvma@gmail.com. Patch usage fees are paid to the State Quartermaster as described above. They are paid to the State PayPal account at txcvma@gmail.com. PayPal to PayPal: Use FRIEND AND FAMILY payment option to ensure no PayPal fee is removed if transferring the funds from another PayPal account. Credit/Debit Card through PayPal: Add an additional amount to cover the fee that will be taken by PayPal. The fee for each transaction is 2.9% plus$0.30of the amount that is received. $20 dues = $0.88 PayPal fee (Send $21.00) $10 dues = $0.59 PayPal fee (Send $11.00) $200 LM dues = $6.10 PayPal fee (Send $207.00) $100 LM dues = $3.20 PayPal fee (Send $104.00) $50 Patch Usage Fee = $1.75 PayPal fee (Send $52.00) $40 Patch Usage Fee = $1.46 PayPal fee (Send $42.00) 8 20 May 2020 REVISION 1.5
The DD-214 or ERB/ORB It is your duty and responsibility to safe guard every applicant s Personally Identifiably Information (PII). Always ask an applicant to submit a DD214 or ERB/ORB with their SSN and DOB redacted (blacked out). We don t need that information for anything so it s best that we never have received it. 9 20 May 2020 REVISION 1.5
The DD-214 or ERB/ORB Be vigilant against fake documentation. Stolen valor is very real. Here is an example of a fake DD214 (expand the image if needed). Notice the different fonts, breaks in awards, and combat patches are not annotated on a DD214. Active Duty Documentation Active Duty documentation will include the following as applicable to service: Active Army can request from Personnel office a copy of their Enlisted Record Brief (ERB) or Officer Record Brief (ORB) Enlisted active US Navy and Marines need page 4 of NAVPERS 1070/604, Enlisted Qualification History, it shows awards received and dates awarded. (Individual can access Electronic Service Record (ESR) and download). Officers active US Navy and Marines need NAVPERS 1070/123, Officers Summary Report and NAVPERS 1070/613, Administrative Remarks which identify awards received (i.e. GWOTEM, Campaign medals, etc.) (Individual can access Electronic Service Record (ESR) and download). Active Duty USAF and recently retired can access this website http://www.afpc.af.mil/ to get the USAF awards page, which shows all awards an Airman has received. 11 20 May 2020 REVISION 1.5
Qualifying Awards This is not an ALL INCLUSIVE list of qualifying awards but it can be used as a quick guide for checking qualification. Always check with the TSR if you have any questions.
