Student Health Insurance Requirement All students enrolled in a qualifying program who registered for six or more credit hours including co-op students on work quarter, are assessed a fee for single student coverage under the university s student health insurance plan. Students are given the option to waive this coverage, if they have coverage that meets the requirements.

  3. Student Health Insurance The University s Student Health Insurance Plan covers major medical expenses, and is underwritten by United Healthcare Student Resources. The plan includes in-network and out-of-network coverage, and is gold tier standard. The Student Health Insurance plan is compliant with the Affordable Care Act, and has a huge nationwide network of providers available. Additionally, those insured by the Student Health Insurance plan have additional benefits when seeking services through the University Health Services (UHS).

  4. Student Health Insurance Premiums $1232/semester Charged to the bursar bill in the Fall and Spring semester $597 charged in summer; only if you did not have coverage in the spring semester Family plan coverage is available for an increased premium.

University Health Services Benefits under the Student Health Insurance Plan No charge for primary care Primary care extends beyond preventive services to include women s health, lab work (medically necessary), x-rays No limit on primary care visits to UHS Mental Health Visits are < $20 per session Prescriptions are $15 for generic, and $30 for brand name, $60 for brand name when generic is available.

Coverage outside of UHS A referral is required before seeking non-emergent care or treatment outside University Health Services (UHS) to ensure the highest level of coverage is received through Student Health Insurance. This does not apply if the insured is: more than 50 miles from UHS receiving out of network coverage for ob/gyn or maternity care

  10. Global Emergency Services For international students you are eligible to receive these services worldwide, except in your home country. For domestic students you are eligible for these services when you are 100 miles or more away from the University and 100 miles or more away from your permanent home address, while participating in a Study Abroad program. All services must be arranged and provided through United Healthcare Global services to be considered for payment. If the condition is an emergency , you should go immediately to the nearest physician or hospital without delay, and then contact the 24- hour Emergency Response Center.

  12. Dental and Vision Coverage Routine dental check-ups and eye exams are NOT part of the coverage through the Student Health Insurance.   14. Waiving Student Health Insurance STUDENTS MUST SUCCESSFULLY OPT OUT OF COVERAGE BY THE DEADLINE TO AVOID THIS FEE. Deadline for Fall 2018 is September 11, 2018

  15. Waiver Process Waivers are completed online by logging into Catalyst.uc.edu You will be re-directed to Gallagher s web page in which you will input your personal information. You will receive an email confirming that the information has been received. You will receive a 2ndemail confirming that the waiver was either approved or denied. If denied, follow the directions on the email for re-submission. Always review your student Bursar account for correct charges.

  18. Getting your records into Med+Proctor Step 1: Make sure you have a final copy of your Immunization Form from CastleBranch including all lab reports/chest x-rays. Step 2: Respond to the email invitation* sent to you by Med+Proctor (M+P) and register for an on-line account. *This invitation will be sent to your official UC email address. http://www.medproctor.com/

  19. Step 3: Once you have registered in Med+Proctor, complete the on-line TB screening questionnaire TB screening found under the Immunizations tab.

  20. Step 4: Find Your Requirement Group All incoming undergraduate, graduate and professional students who are registering for > six [6] credit hours must comply with this policy. Over the summer, the UCCOM Office of Admissions helped make sure you met these requirements in time so that you didn t miss any clinical activities by working with you through the Castle Branch portal. The form that you completed in Castle Branch IS your Med+Proctor form! In M+P, the College of Medicine has its own specialized form. To find it, begin by selecting General Population
