Background
 
Leicester City Council Changing Futures Programme
Engaging with people facing multiple disadvantage
including:
Women with multiple disadvantage, particularly sex
workers
People experiencing homelessness
People who have had contact with Criminal Justice
System in previous 12 months
Those with Dual Diagnosis (co-occurring mental illness
and substance misuse) or Proposed Dual Diagnosis
Complex Mental Health Cases

The Aim
 
To provide intensive support to individuals in order to
make progress with them at an individual level.
To facilitate access to other services (drug treatment,
social care etc) through the specialist team members.
To identify barriers within the systems that prevent the
progress of individuals facing multiple disadvantage.
To develop new pathways which enable people to
make progress at the point when they are ready to
access a service (reachable moment), in order to
overcome the barriers that have been identified.
 
How We Work
 
Partners submit referral through online portal.
If the Changing Futures criteria is met, they will be allocated an
intensive support worker.
In collaboration with referrer, goals and aims for work with the
individual will be developed.
The intensive support work will take place in collaboration with
partners and supporting agencies with a pathway back to the referring
agency (or a different agency if a more appropriate agency has been
identified) at the end of the period.
An exit plan will be proposed at the point of referral i.e. the referrer
will identify what progress they would hope to be achieved within the
period, in order for them to be able to continue making progress with
the individual in the long-term.
 
Starting Point
 
The Changing Futures model is based on the
success of the Council’s Frontline Street
Lifestyle Partnership which received
recognition for the strategic approach taken
with respect to the complex issues of:
Begging
Street Drinking
Street Based Drugs Misuse
Rough Sleeping
 
Multi-Agency Partnership
 
A multi-agency partnership has been
established to enable information-sharing and
collaborative working around each individual.
This collaborative working will also provide an
overview for Changing Futures so that the
systems barriers can be identified, and a
pathway can be developed by the agencies
that will be providing the services.
