Moda Health OEBB Wellness Funds Wellness funds are for entities seeking support with their workplace health and wellness efforts An entity may apply for up to $10,000 per calendar year Funding is awarded on a first qualified, first served basis Priority is given to new applicants and applicants not currently funded through other grant or funding programs

3. Wellness Funds Examples Wellness spaces Nutrition workshops Fitness classes and/or equipment Corporate meditation app access Activity challenge platforms Healthy snacks Social events

4. Guidelines All employees must benefit equitably from the funds Must complete employee interest survey run by Moda Health to be eligible Funds are not eligible for use as cash, gift cards, or incentive-related or individual-based rewards, lodging, decorations, employee salaries, professional development or continuing education courses, and items to restock classroom/staff/meeting lounges All federal and state statutes/regulations and entity regulations need to be followed Submit an evaluation form within 30 days of utilizing all awarded funds
