  4. Near Miss Reporting Tool Launched on the 10th December 2015. Salim Sikaiti, GD as the Champion. Had good response from staff and contractors. About 60-70 reports per month. Had to filter as some of the reports are unsafe acts/conditions and feedback to the sender.

Easy access and user-friendly reporting tool Employees are encouraged to report near misses without fear of discipline or loss of job. Reporting and tracking near misses can provide valuable information as to where weaknesses in the safety program exist. Enables companies to pro-actively control/eliminate hazards before a tragic or costly incident occurs.

CHALLENGES The Near miss reporting tool has become a one stop-shop for ALL types of incidents and unsafe acts/conditions. There are cases where No actions even recorded in the report, hence, losing the opportunity to learn and opening the chance to happen again. HSE Team Leads are not immediately informed and real time cascade of learning is lost.

QUICK FIX A simple explanation of the difference amongst Near miss without consequence; Incident with consequence; and Unsafe act/condition. Make the Corrective Action Taken as a mandatory field to be filled up. As this will create a close loop of the near miss report. HSE Team Lead to be notified when there is a near miss reported under their directorate. We will continue to advise staff and HSE TL of near miss reports with high potential Red ram and Yellow 5A/5B to be investigated and reported in PIM. Involve MSE3 team for lateral learning taken from near miss reports.

Post Implementation Review Team will be formed to conduct a review of the system where actions can be developed to sustain the success of the reporting tool.

  11. In Need of Your Support Refresh your team of the types of incidents including Near Miss; and the Incident notification and reporting process. Encourage your team to discuss near miss, immediate cause and its corrective actions in daily safety meetings. Identify blockers of not reporting incidents including Near Miss. Share and use the new reporting tool to your team.
