Check SPAM folder to allow Correspondence Conditional Award Notice from Submittable Requests for all steps of the application process are sent through Submittable messages Application Approved Notice of Denial Reason for denial is provided and supported by the MCA/ARM Application does not move forward Application Denied

  5. Next Steps if Application is Approved You will get a message from Submittable notifying you of award in full or a reduced amount (as determined by review team). Approved You will receive a secondary message with instructions to complete the required financial agreement access to online additional forms Request for W9/Grant Terms This step must be completed in 7-days from award notification or funding is forfeited Deadline

  7. Training Dates Attend or Request Amendment Ensure start and end dates remain as proposed. No changes, proceed with completion and prepare for Reimbursement next steps. Attend Training Write an email to the program manager and request a need to change the start date or end date or both. Dates Changed Complete an Amendment form sent from Submittable to support training date change. Have the trainer provide supporting documentation of revised dates. Required for adjustments as verification. Complete Form

  9. Expenses Attendees Time You have 30-days after training completion to submit your claim for reimbursement. Individuals must complete training and have proof of completion provided by the trainer listing all eligible individual names in attendance. In order to be reimbursed for your full award, you must provide proof of expenses that support your awarded amount. Preparing your claim A due date was assigned upon receipt of your financial agreement. Note: the reimbursement cannot exceed the appropriated award. The award amount, however, can be reduced based on actual expenses or incompletions by attendees. You will note any individuals who did not complete from your proposed Employee Roster of eligible attendees. Only one reminder will be sent

  12. Post Evaluations are due 6-months after training. Measuring incorporation into your operations and not an assessment of the trainer
