  1. Wellness Assessment In the following pages you will find a self assessment designed to gage your current state of wellness. Answer each of the following questions from 1-10, with one being never and 10 being always. Based on your results, color in each section of the Wellness Wheel for a visual representation of your overall wellness.   2. Physical Wellness Promotes proper care of our bodies for optimal health and functioning 1. I exercise regularly. 2. I eat a nutrionaly balanced diet. 3. I get regular medical checkups. 4. I am satisfied with my outward appearance. 5. I am intentional about the risks I take. 6. I engage in healthy self care. Score Scored 1-10 1 = Never | 10 = Always Total Emotional Wellness The ability to be aware of and accept our feelings rather than deny them. Having an optimistic approach to life. The capacity to enjoy life despite its disappointments and frustrations. 7. I can identify and understand my emotions. 8. I can appropriately express my emotions. 9. I can recover well from negativ e experiences. 10. I am adaptabel and adjust well. 11. I recognzie when to seek the help of others. 12. I engage in activities that are emotionally satisfying Score Scored 1-10 1 = Never | 10 = Always Total Spiritual Wellness A personal matter involving values and beliefs that provide purpose to our lives. 13. My life decisions ar in line with my value system. 14. I can define my life's purpose. 15. I am generally at peace. 16. I enngage in activities that are spirtually satisfying. 17. Intellectual Wellness Score Scored 1-10 1 = Never | 10 = Always Engaging the individual in creative and stimulating mental activities to expand knowledge and skills and help them discover the potential for sharing their gifts with others. 19. I am open to new concepts and ideas. 20. I engage in intellectual conversations with others. 21. I enjoy sharing things I have learned. 22. I find ways to be creative. 23. My goals are relevant and achieveable. 24. I desire to grow both personally and professionally. Total Occupational Wellness Involves balancing school work, jobs and leisure time. Learning ways to reduce stress and joyfully live within our means.sharing their gifts with others. 25. I maintain a balance between work/academics and personal life. 26. I am satisfied with the directin of my career/profession. 27. My career/profession adds joy to my life. 28. I live within my means. 29. I have a financial plan for my future. 30. What I am doing professionally adds meaning to my life. Score Scored 1-10 1 = Never | 10 = Always Total Social Wellness Refers to the relationships we have and how we interact with others. Social wellness involves building healthy nurturing and supportive relationships as well as fostering a genuine connection with those around you. Score Scored 1-10 1 = Never | 10 = Always 31.
