Primary Care Networks (PCNs) play a crucial role in healthcare by focusing on community health, managing long-term conditions, ensuring timely access to services, and promoting sustainability. Key PCN priorities include healthier communities, well-managed patient care, integrated working, and innovative solutions. PCNs recruit a range of roles such as Nurse Associates, Social Prescribers, Clinical Pharmacists, and more to provide comprehensive care. National specifications require PCNs to deliver services such as enhanced hours, Covid vaccinations, and research projects. Effective patient communication through newsletters, websites, and community events is essential for promoting services and engaging with the community.
