  2. Who WISe Serves Medicaid Eligible Youth ages 0-21 With behavioral health challenges impacting multiple areas of functioning (meets CANS algorithm) Often multiple systems are involved (ie: special education, juvenile justice, DCYF, etc.) though not a requirement

  3. Core Values of WISe Family and Youth Voice and Choice Team Based Strengths Based Culturally Relevant Home and Community Based Natural Supports Individualized Collaborative Outcome Based Unconditional

  4. The WISe Team Peers Use their personal story of recovery to join, build connection, break isolation and stigmas, and foster hope. Clinician Provides behavioral health interventions in the form of individual and family therapy using a wide variety of modalities tailored to the individual needs of the youth and family. Intensive Care Coordinator Partners with the youth and family to develop and strengthen their team of supports. Facilitates monthly Child and Family Team Meetings and ensures all parties are working from one plan.
