ValueCares pilots 7 large-scale pilots to test the digital solution in Europe Pilot Target Older citizens that had a heart attack and finished rehabilitation Croatia(Rijeka) Frail older people with neurological problems (Multiple Sclerosis, Parkinson s, Stroke) with or without co-morbidities after hospitalization Greece (Athens) Older people (75+) with mild to moderate frailty Ireland(Cork/Kerry) Older people with diabetes or cardiovascular diseases or mild cognitive impairment Italy (Treviso) Older people who have had a stroke The Netherlands (Rotterdam) Frail older people with no or mild cognitive impairment with comorbidities and lack of social and family support Portugal (Coimbra) Older citizens with mild to moderate frailty Spain (Valencia) @valuecare_eu projectvalueCare.eu
Co-design activities Collective creativity, applied across a design process, of different actors working together in a specific design development process (Cooper et al., 2013).
