Variety Care Farm Worker Health Program Overview
Variety Care manages the Migrant and Seasonal Farmworkers Program with success since 2001. About Us Variety Care assumed the responsibilities for the Migrant and Seasonal Farmworkers Program in 2001 and has since operated it with great success. We are a non-profit Federally Qualified Health Center and receive funds from HRSA to operate the Migrant and Seasonal Farmworker Health Program.
History This program, funded by the Health Resources and System Administration (HRSA), a branch of the U.S. Department of Health and Human Services, provides medical, dental, vision, and pharmacy services to program participants. The program is available to migrant and seasonal farm workers from other countries regardless of their immigration or legal status. Slide 3
Background There are 166 federally funded migrant health center entities, all but 15 of which are jointly funded Community and Migrant Health Centers, and those 15 are generally known as "Voucher Programs". Slide 4

ELIGIBILITY REQUIREMENTS Who is Eligible? 1. Primarily employed in agricultural work within the last two years (24 months) making up a significant portion of a permanent job or as a full-time seasonal job. a. Agriculture worker cannot be self-employed Patient completion of a status verification form (Migrant, Seasonal, Homeless, Public Housing, Veteran, etc.) Patient must provide verification of insurance OR income status if uninsured Family members of the agricultural worker, broadly defined as: a. Immediate Family (mother, father, son, daughter, etc.) b. Inclusive also of extended family (grandparents, domestic partners, common law relations and extended family living together) 2. 3. 4.
Sliding Fee Scale We accept SoonerCare (Medicaid), Medicare, and many other insurances. For people who do not have insurance, we determine the cost of an office visit based on household income and the number of people in the household.
Services The purpose of this program is to assure access to primary health care for low income and medically underserved migratory and seasonal farmworkers (MSFW) and their immediate families. To address the serious health needs of these workers and their families, the program will provide vouchers for the following care: Medical/Behavioral Health Dental Care Vision Care X-Rays Pharmacy (The funding of the program is not unlimited so there have been set limits on the program which will be provided from Variety Care to our access point providers on a regular basis if changes are made. )

In the last two years, have you or anyone in your family worked in any type of agriculture (farm work) like: planting, picking, preparing the soil, packinghouse, driving a truck for any type of farm work, working with farm animals such as cows, chickens, etc. Yes or No 2. In the last two years, have you or a member of your family lived away from home in order to work in any type of agriculture (farm work)? Yes or No 3. Have you or a member of your family stopped migrating to work in agriculture (farm work) because of a disability or age (too old to do the work)? Yes or No
FAQs #2 Agricultural Workers or Dependents: Definitions Migratory: An individual who establishes a temporary home for the purposes of such seasonal employment, including aged or disabled former agricultural workers Seasonal: An individual who does not establish a temporary home for the purpose of seasonal employment. Status must be verified at least every two years.
Area Coverage The service area for this program will be the Southern half of Oklahoma following Interstate 40 from the Texas border to the Arkansas border and everything South of Interstate 40.
What The Program Does NOT pay 1. Hospitalization or In-Patient Services 2. Emergency Room Services (not provided under an agreement of an access point) 3. Extensive treatment or unnecessary treatment 4. Diagnostic testing (aside from X-rays) 5. In home care or in home care supplies 6. Extensive dental services beyond basic dental care 7. Other services which exceed the parameters of the program.
