Fraternal Programs Report Form #10784
FRATERNAL PROGRAMS REPORT FORM (#10784) Use this Supreme form to report all your program activity each period. Due each period (Sept 15, Dec 15, March 15). Submit on-line! GK should email completed form to his DD and to Peter O Sullivan.
Use this link to pull the report from Supreme: https://www.kofc.org/en/forms/spa/invite.html?lang=en&form=10784C.01 This is the message you will receive. Enter your Membership Number and your Last Name, then Proceed.
Within seconds, you will receive these messages which enable you to access the form:

State Council Service Program Awards Entry Form #STSP
STATE COUNCIL SERVICE PROGRAM AWARDS ENTRY FORM Use this form only to submit your 4 best programs- one per program category. Form serves as your cover sheet to write a report up to 5 pages in length. These reports will be judged for State recognition awards.
