Following information about your facility need to be provided Name of your Facility Address of your Facility Facility Phone Number Contact Name and Email Facility Type CLIA Reach out to OSDH to start the onboarding and testing Request for a SpringML account/credentials ELROnboarding@health.ok.gov You will receive a password reset email to reset password and login You will receive documentation if you would like to connect via REST API Provide Additional Information about your Facility Facility Type, EMR/LIMS used, Option Chosen, Tests Performed, LOINC and SNOMED 6
