Adult Services Adult Services Partnership Progress/Developments in Last Quarter Key Objectives for Next Quarter BCC Section 75 Near completion of the work to bring in the out of hours AMHPs into the S75 (to join the long standing inclusion of the day time AMHP service) creating one 24/7 AMHP service within the S75 in Bucks. Engaging with the work to develop the new operating framework within Adult Social Care. Creation of new Social Care Pathway for placements and social care only cases OCC Section 75 Section 75 agreement remains in place New schedule of JMG meetings to be confirmed for this year Refresh of annual objectives to be confirmed Address issues identified in the recent audit and develop the business process within the S75 IAPT Oxon (PML/Mind) Contract with OCCG (5 years) signed Key national targets met Ensure we remain on track to deliver contractual and national targets Sub-contracts with PML and MIND being drawn up and to be signed in quarter Healthy Minds Bucks (Richmond) Planning to expand work with Richmond Fellowship in Bucks as we have put a bid in (with CCG) for work with DWP proposing to work with job centre staff aiming to recognise people with MH issues. Obtain advice from legal and procurement on partnership arrangements to ensure compliance with public sector contract procedure rules Live Well Stay Well Healthy Minds Bucks (Bucks Healthcare) Way Forward with (BHT) Development of Live Well, Stay Well contract with BCCG with a view to further rollout of the programme across the county Working with Parkwood to deliver the Live Well, Stay Well programme Sub-contracts with Parkwood, BHT and CV Health to be drawn up and signed in the quarter Caring, safe and excellent
Adult Services Adult Services Partnership Progress/Developments in Last Quarter Key Objectives for Next Quarter Relate Bucks Formally accepted across from CCG to Oxford Health in contractual terms Head of Service reviewing contract performance OMHP Oxon (Mind, Response, Restore, Connection FS, Elmore) Agreement to make MIND information workers and Connection FS worker posts permanent in AMHTs Recovery College funding being met from Directorate budget as no additional funding for this project ( 250k per annum to Restore who host it) Response Project 10 now fully operational, positive impact reducing delays in Oxon wards by moving on 10 patients (cost 100k) Response CEO chair of PMG Operationalising project work-streams which will now feed into the OMHP SMT Single point of access work-stream should be completed this next quarter (delayed from previous report) Recovery College new prospectus to be published OCCG commissioners will attend first PMG this quarter Appoint partnership manager to lead on partnership developments , including recovery colleges. Urgent Care Mental Health (SCAS and TVP) SCAS mental health nurse scheme now established at ambulance control Street Triage with TVP fully operational in both counties (two centres in Bucks and one in Oxon) Both successfully working with TVP in providing appropriate mental health crisis response PIRLS and EDPS both providing 24 hr care liaison psychiatry within BHT and OUH hospitals Both meeting their response targets and both being seen positively by acute provider colleagues Prepare 6 monthly report in July Trust to be represented at A&E performance meeting which is replacing SRG in Bucks Caring, safe and excellent
Adult Services Adult Services Partnership Progress/Developments in Last Quarter Key Objectives for Next Quarter Thames Valley and Wessex Forensic Network New Care Model application successful following interview Initial network meetings / workshop complete (August & Sept) Financial and patient level data received for 15/16 and snapshot (June 2016) Initial reviews of data conducted Further meetings with NHS England (region and centre) to agree actions/next steps. Board approval for New Care Model to begin in shadow form from 1st October (with no financial risk, excluding TCP patients) Full due diligence to validate and reconcile patient and financial information Further network meetings (Monthly) Establish governance arrangements for NCM Conduct initial reviews of patient cohort to prioritise pathway developments and actions Review contracts with NHS England to agree actions and changes Business case developed (for approval in December) (OHFT, Response, Southern Health NHS FT, Dorset Trust, Isle of Wight NHS Trust, Solent NHS Trust, Berkshire NHS FT , Central & Northwest London NHS FT) Partnership with Restore (Patients shop at Littlemore) Discussions had regarding ensuring there is ongoing patient participation in staffing the shop. Patient feedback provided to Restore regarding the interview process and that patients would prefer not to be interviewed in a group. Agreement made with the Restore CEO that out of county patients can attend the Restore shop prior to funding being agreed. Quarterly meetings held between Restore and OHFT forensic staff with a view for ongoing monitoring of all attendees. Promote the opportunities provided by the shop, to the patients, at Ward Community meetings. Information from quarterly meetings feedback to FSM regarding attendance, individual progress and funding. Older Peoples Services Older People s Services Partnership Progress/Developments in Last Quarter Key Objectives for Next Quarter Acute (OUH) Operational - Updated choice policy agreed and implemented for Oxfordshire Operational continue joint work on choice and patient flow through community hospitals to ensure consistency of approach across all settings (acute and community hospital) Acute (RBBH) Delivery of MIU and GP OOH at Henley Townlands working smoothly. Due to soft launch RACU in November. Successful launch of RACU in November Various SLAs (OUH) AHP Radiography Community hospital medical cover Positive feedback from medical colleagues on CQC improvement actions for community hospitals (e.g. pain assessment documentation) Strengthened quality review processes working well (OUH medical cover for community hospitals) following learning from Sis Clinical audits and review as planned Age UK Oxon Generation Games are working with OH falls service to develop and deliver a range of exercise programmes focusing on strength and balance to reduce risk of falls Review opportunities for directly managing contract for exercise programmes to reduce falls risk GP Federations (Oxfordshire) Agreement in place with all Oxon GP federations to explore joint enterprise model to deliver full multi-disciplinary primary care (fruition of locality / neighbourhood integration) to maximise support to patients and reduce pressure on GP and community workforce Define options for organisational form; and proposals to shift support to primary care neighbourhoods / localities Hospices (Oxfordshire) Agreement in principle to explore joint pathway for EoLC domiciliary care for patients receiving Fast Track CHC funded care. Review current contract value Barnardo s in Buckinghamshire CAMHS 1 years notice given (1st July) on contract due to very poor update of service for both YP and parents. Developing training programme for next nine months and scoping possible partnership across the age range. Beat in Buckinghamshire CAMHS Delivering against current contract due to expire March 2017 Decision required whether to extend linked to SWB CAMHS retendering timescales Relate Wiltshire (Banes and Wiltshire) Consider potential for cross border Executive functions Local Safeguard Children s Boards Oxfordshire Buckinghamshire Wiltshire Swindon BaNES Consultation with Board members following Wood report to determine focus and structure of local Boards Increased requirements for performance data and audits Impact assessment and planning due to reduction in youth justice funding, and reduction/reconfiguration of services in Local Authorities and Police Youth Offending Boards Oxfordshire Buckinghamshire Wiltshire Swindon BaNES Refine service delivery and key objectives in all affected areas Caring, safe and excellent
Key Corporate Partnerships Key Corporate Partnerships Partnership Progress/Developments in Last Quarter Key Objectives for Next Quarter CLAHRC Patient experience based co-design in early intervention in psychosis services (EBCD in EIP): Healthtalk module is progressing well: Fauzia Knight (post doc) is now in post and will be undertaking the work to produce the module. CLAHRC Symposium likely to take place in the Autumn. Preventing falls and fragility fractures: Slippage in the project is due to a key staff member leaving to take up a studentship. This has been discussed with the Theme Lead and the project and associated funding re-phased. New appointment is underway Optimising Treatment for Mild Systolic hypertension in the Elderly (OPTiMISE): Additional funding has been secured from the NIHR School for Primary Care Research to perform concurrent qualitative and economic evaluations which has resulted in a change to the milestone dates. The project lead is aware that CLAHRC funding ends on 31/10/2018. The project is on target to meet the revised milestones. Blood pressure self-monitoring for the management of women during pregnancy with chronic hypertension: a feasibility study (OPTIMUM-BP) PHASE 2 : Project ahead of schedule. Recruitment began in January 2016 NIHR Clinical Research Facility Full CRF application to renew and expand facilities further to OUH has been submitted, including video. No requirement for an interview Waiting to hear back whether application has been successful. NIHR Biomedical Research Centre Full BRC application and video have been submitted to NIHR. Preparations continue for interview to held in July. Discovered OHFT bid was successful and now implementation plan will be developed Caring, safe and excellent
Key Corporate Partnerships Key Corporate Partnerships Partnership Progress/Developments in Last Quarter Key Objectives for Next Quarter OUHT.OHFT Joint Research Office Work is continuing trying to establish methods of working across organisations. Milestones have been revised due to delays with national HRA processes and results form CRF and BRC applications will not be available until Sept 2016 which will impact on this going forward Work ongoing to establish ways of working now there are 2 BRCs in Oxford.
