  6. Elimination of Fatalities and Injuries Review regulations of MHSA pertaining to duties, appointments and responsibilities of rock engineers. Inspectorate and 12.1 appointees must be capacitated and the functions of 12.1 appointees be clarified. MHSC to serve as an information hub for the mining industry and disseminate all fires and explosions research and outcomes and liaise with MQA regarding training. 70-100% Review legislation pertaining to fires and explosions. Elimination of fatalities and injuries Expedite 25-69% Develop a programme of training and empowerment of Health and Safety Representatives and Supervisors. Graduate development to train and upskill rock engineers and seismology specialists. <25% Conduct a study to determine the optimum ratio of Rock Engineers to mines. Develop promotional material to highlight the risk relating to mining of pillars. Investigate availability of technology/system to facilitate real time seismic rating. Popularise promotional material on early entry examination. Identify and provide training programs to identify hazards, assess; respond to risks and implementation of CoP. Conduct a study regarding accountability for decisions taken that lead to future disasters or accidents. Recommendations from research done on mining patterns in narrow reef deep level mining. Consider research and possible purchasing or development of portable machinery for monitoring seismicity. Assess the feasibility of utilizing independent investigators for incident investigations at mines. 25 years in Pursuit of Zero Harm

Elimination of Lung Diseases Encourage collaborative efforts and a multi-sector approach (including researchers, legislature, specialists, management, organized labour, supply chain personnel and OEMs. Prioritize vulnerable groups and missing cases through targeted awareness campaigns. 70-100% Stakeholders to engage with other countries where noteworthy achievements have been made in mining health (in order to try to understand their procedures from the beginning to end and adopt those within the context of South Africa towards a safe and successful mining industry). Elimination of Lung diseases Expedite 25-69% Mines to consider hiring independent Occupational Hygienists and Medical Practitioners, and data to be verified independently. <25% Appointment of two or three health inspectors per province for OLD. OLD milestones to be enforceable and regulated and capacity of inspectorate and Hygienists to be increased. Comprehensive training of workers on airborne pollutants exposure and OLDs and sharing of knowledge and adoption of leading and best practices. 25 years in Pursuit of Zero Harm

Promotion of access and provision of treatment in a dignified manner that respects human rights. Equip leaders to understand the implications of technology decisions in making the working environment safer and healthier. Review Bonus system to ensure that representatives prioritise health and safety. Draft legislation that will enforce executives liability and accountability with regards to injuries and fatalities. Use Regional Tripartite Forums as a platform to drive cultural transformation and mine health and safety days. Accountability measures must be put in place in order to begin to inculcate a culture that is enabling to the guidelines of RRDW and RRDWL. Provide knowledge to strengthen knowledge of MOSH leading practices to empower employees through their representatives. Improve tripartite relationship between government, labour and the employer. Training and awareness of employees on major health and safety hazards. 70-100% MHSC to work with MQA to develop a programme to enhance the skills set identified for the SAMI.   13. Conclusion 95% achievement against 2014 initiatives. Most of the 2016 pledge initiatives are at 100% implementation. Focus on implementing the health and safety awards 51% achievement against 2018 initiatives, with just over 30% that are below 25% implementation. Good progress made by the council and its tripartite stakeholders, but there is still much work to be done to achieving our milestones and goal to zero harm, especially with regards to our elimination of fatalities and injuries and the culture transformation focus areas. With 2 years to 2024, how do we use the breakaway sessions to strategically think about how we could accelerate the implementation of current initiatives, as well as being agile enough to address emerging risks. 25 years in Pursuit of Zero Harm
