Accomplishments Accomplishments WIC Breastfeeding Initiation Rates has increased from 80.0% to 82.5% which exceeds the Healthy People 2020 goals WIC Food Benefits brought $5.9 million into the Lake County economy in 2018 EH scored 96% on their Evaluation on Facility and Water Program In collaboration with the Lake County Schools Student Services Department, the school health team provided immunizations to 6th grade students at the area middle schools. 4

Vital Statistics Vital Statistics The Bureau of Vital Statistics at the Florida Department of Health in Lake County (DOH-Lake) in partnership with The Lake County Tax Collector s Office, will allow customers to obtain birth certificates at three new locations. Birth certificates are available for all Florida births from 1930 to the present. The cost at the Tax Collector s Office is $15.00 for each certified copy, additional computer copies for the same person is $10.00 each, with a $6.25 processing fee. Only parents, legal guardians, or the person whose name appears on the certificates, if they are at least 18 years of age, can receive a birth certificate.   8. Public Health Preparedness Public Health Preparedness DOH-Lake assisted with state and nationwide hurricane response. Three DOH-Lake staff were deployed for one week as part of response efforts for Hurricane Michael. In addition, DOH-Lake s Administrator was deployed for two weeks as part of the effort to respond to Hurricane Florence in North Carolina. DOH-Lake has incorporated lessons learned from Hurricane Michael and the experience of counties in the Panhandle to improve hurricane response locally. DOH-Lake is now prepared to staff and supply up to five special needs shelters. 8

  9. Public Health Preparedness 20 new volunteers have been added to the Medical Reserve Corps roster. MRC volunteers assist with preparedness activities, outbreak response, and emergency activations. The Department is also improving partnerships with local organizations for mass vaccination and medicine distribution efforts and has secured key partnerships as part of our Closed Points of Dispensing (POD) program. DOH-Lake has developed a Closed POD training guide and informational mailers to encourage participation in the program 9
