Learning Objective Understand a framework for system and data integration to prevent and end homelessness, which includes three core components: 1) Cross-sector change leadership 2) Data integration 3) Data maturity (for data based decision-making) Understand how to apply this framework through critical lessons learned from communities

How can we create stronger cross-sector referral networks and identify eligibility? How can we prioritize and target the most vulnerable persons accessing services?
