Process Mapping: PREWORK Pick the Program: Or resource with the most significant bottlenecks or areas for potential improvement Identify the Perspective and Users: Identify if you will be improving a system from the perspective of the process administrators or the process users. Select the people that are most familiar with the current program or resource who will prototype an improvement plan from that perspective.

Agenda Process Mapping: PREWORK The MOCK Process Mapping Meeting Process Mapping: THE BIG RED BALL HOUSING PLACEMENT
The Mock Process Mapping Meeting Agenda: 1. Identify the AIM (Objective) for the meeting Improve the VASH Housing Placement through our local Coordinated Entry System VASH VASH HOUSING PLACEMENT
The Mock Process Mapping Meeting Agenda: 2. Define the Process Start and End Improve the VASH Housing Placement process from MATCH to HOUSING PLACEMENT or MOVE-IN VASH VASH HOUSING PLACEMENT
The Mock Process Mapping Meeting Agenda: 3. Identify the Measurement: How will we know a change is an improvement? We will know if a change is an improvement by measuring the # of days from MATCH to MOVE-IN VASH VASH HOUSING PLACEMENT
The Mock Process Mapping Meeting Agenda: 4. Map the Current Process and layer in current measures/data. 5. Identify the Bottlenecks 6. Brainstorm Change Ideas / Select Ideas to Test 7. Set Up the PDSA 8. Next Steps / Closing VASH VASH HOUSING PLACEMENT
Improvement Project Management 1. Identify the Improvement Project Manager - Designate who is holding the big red ball and will convene future meetings and drive the improvement team forward 2. Identify the Timeline: Decide when the improvement team will meet again (at what intervals and for how long) to evaluate and measure if the ideas being tested are resulting in process improvement. 3. Assure that Project Management includes continual data analysis to understand where to focus improvement efforts - when to stop doing something, when to modify the idea, when to scale and codify!
