Individual clients in crisis are assigned a team
Will only form if the client consents and signs an 
authorization

Comprised of member of all disciplines the client is engaged with.
Develop support plans, check in regularly, become resource
Facilitates communication and earlier intervention when issues are
presenting
This has transition to the crisis response process
CSG / PERF DATA COLLECTION PROJECT
Data Sharing as an Early warning system
Goal – to reduce numbers of repeat callers, and to identify people at risk
and make resources available sooner.
Reduce officer contact, reduce the escalation of crisis and the potential for conflict,
injuries and liability
Key project accomplishments
Change code to BH (Behavioral Health) from Mental Health
Added CIT Disposition to CAD
All CIT dispositions compiled and sent to Central Ohio Mental Health each morning
CoMH offers services
This has now evolved with our addiction response to a CIT/RES referral.
Data Sharing Outcome
Results
High percentage of identified persons were already known to mental health.
Many newly identified persons declined services, but some did accept and
some later called back.
Lower frequency of calls from a handful of known frequent consumers
Lower frequency of calls from persons in crisis before intervention occurred.
Involved with Attorney General’s CIT Data Collection Project.
Ongoing work with Criminal Justice Coordinating Center of Excellence,
Northeast Ohio Medical University.

Data sharing in the drug and alcohol addiction realm
Designed to work with the Rapid Engagement Specialist at Maryhaven
Service Coordination
Funded and Hired in cooperation with the Delaware
Morrow Mental Health Recovery Services Board.
Accept and process referrals of individuals and
families from the CIT (Crisis Intervention Training)
and RES (Rapid Engagement Services) Data
Sharing process of the Delaware City Police
Department, as well as directly from law
enforcement when appropriate.
Actively manage CIT Data Sharing reports, including
reviewing referrals and screening for appropriate
behavioral health and/or healthcare linkage.
Facilitate follow up to provider system when
individual is not currently connected to services.
Provide service coordination including screening,
intake, education, advocacy and referral of
individuals and families in crisis to service providers
in the community.
Provide screening and initial intervention, determine
behavioral health needs, and connect identified
individuals to appropriate care when needed.
Accurately assess needs and link the individual to
appropriate community resources.  
Provide ongoing follow up and linkage services to
referred individuals until actively engaged in services
or immediate needs are met.  Provide short to
intermediate term case management when
appropriate.
Develop and maintain positive working relationships
with community behavioral health and other
healthcare providers, local law enforcement, and
other community systems.  Work cooperatively to
ensure that individuals needs are met.
Provide follow up information to referent law
enforcement and EMS when permitted to facilitate
timely and effective response.
Network with community agencies and providers.
Initiate assistance to individuals in building informal
support networks with other residents, family, and
friends.
Attend staff meetings, supervision with immediate
supervisor, training and other required meetings and
events as scheduled
