PART I assessment Community Profile and Health Needs Assessment report (25%) Describe characteristics of ONE community/region in Victoria (such as City of Ballarat, city of Whittlesea, city of Melbourne, city of Hume etc). Describe ONE significant public health concern within the chosen community in the last five years (such as CoVID 19, homelessness, food safety, teenage suicide, substance misuse, etc). Identify and discuss the major social risk factors (non-medical factors) that has contributed to the chosen issue/concern in the chosen community. Identify and describe policies, current community structure and available community resources aimed at addressing this specific public health concern. An explanation of the nurse s role and responsibilities within the community in relations to address and improve the issue. 2
Describe characteristics of ONE community/region What information should be included (if applicable)? What are the key features of this community? Community population: for example, population growth, birth, gender, age distribution, ethnicity. Relevant key epidemiology if applicable: for example, morbidity, mortality, life expectancy, disease prevalence. Social and environment: for example, employment, environment, education level, crime rate. 3
Describe ONE significant public health concern within the chosen community in the last five years Would be mainly focus on epidemiology of the issue within the locality and in comparison to National or State perspective. But also include identification and rationale for the choice (this can be personal so using I is fine). 4
Identify and discuss the major social risk factors (non-medical factors) that has contributed to the chosen issue/concern in the chosen community. A description of the social risks (focusing on social, economic, environmental, and political determinants) within the local community that likely contribute to the public health issue. A critical analysis of social factors, why they are considered the root causes of the issue? Use Social model of health as the context for discussion (not focusing on the pathophysiology and clinical feature of the condition) 5
Identify and describe policies, current community structure and available community resources aimed at addressing this specific public health concern. Identify and describe the service provision within the community aimed at addressing the specific health improvement issue.: Also need to link to national and local strategy towards service provision for the public health issue if available. 6
An explanation of the nurses role and responsibilities within the community in relations to address and improve the issue. An explanation of the nurse role in facilitating access or increasing service coverage. Address nurses roles and responsibilities from primary care level. 7
Some additional notes. -check mark distribution and marking rubric on the syllabus (page 8) first. If the question worth 10 marks, you need to research, discuss and write a lot more questions than those weigh less. -Know the students you are partner with and work with each other. Communicate regularly and check on each others progress constantly.. -Clear on deadlines and responsibilities, who does proofreading, reference check, submission??etc. -If a member provides poor quality of work or little work, make sure the team has strategies in place and record the evidence and let me know. - Check page 6 of the course syllabus for group work marking rules. 8
