We are here as part of the mission of the Maine Shared Health Needs Assessment & Planning Process Project, also called Maine SHNAPP. Our mission is to create the framework and approach for a shared CHNA that [a] provides valuable population health assessment data to interested organizations and individuals, [b] supports public health accreditation efforts, and [c] addresses community benefit reporting needs of hospitals.

Ultimately, the Maine SHNAPP seeks to turn data into action and create the healthiest population in the USA.

Understanding the health needs of Sagadahoc County through a comprehensive Community Health Needs Assessment (CHNA) conducted in Fall 2015 and Spring 2016. The assessment highlights key health issues such as obesity, drug and alcohol abuse, tobacco use, as well as societal challenges including poverty, access to behavioral care/mental health care, transportation, and health literacy. Stakeholder surveys have identified these issues as major concerns, emphasizing the importance of addressing these health and social challenges for the well-being of the community.
