Timeline Beginning of semester prior to taking 4301 (e.g. Spring if you plan to take 4301 in Summer) 1. Determine Eligibility A. Choose a potential Internship Site Secure a mentor internship site. Due 3 months before 1st day of class to assigned faculty 2. Submit Application Form 3. Submit Mentor Survey + Mentor Forms Due BEFORE first day of class 4. Submit Portfolio Resume Cover Letter Job Description Additional materials Due 1-4 weeks into course (depending on semester) as PDF via TRACS Due by 11:55 PM on Fridays following weekly activities 5. Submit Assignments (progress reports and interview to TRACS Due 2-3 weeks before final grades 6. Submit Final Reports (Mentor Evaluation, Self-Evaluation

+Before You Begin Submit mentor contact information to supervising faculty NOTE: For all clinical and child related sites, you may be required to have a criminal background check and perhaps be fingerprinted. Proof of immunizations may be required, as well as TB testing. Many clinical facilities require an affiliation agreement. These take TIME to be finalized. Start this process as soon as you are approved to take the course.
