The British Red Cross Home from Hospital Service, managed by Debbie Hayter, was jointly commissioned by Portsmouth City Council and Hampshire County Council in January 2018. The service aims to support individuals after hospital discharge with various needs including social isolation, welfare checks, shopping, and equipment. By helping individuals gain confidence, self-manage, and reconnect with their community, the service has made a significant impact. Through collaborations with hospital teams and community resources, the service provides immediate support and longer-term interventions to address individual needs. The case studies highlight the personalized assistance provided to people in vulnerable situations. Looking ahead, the service has been recommissioned until June 2020, with plans to strengthen relationships with the community and healthcare providers. Comprehensive evaluation is also set to be conducted to further enhance the service's effectiveness.
