  3. AHFA DMS must be updated on a monthly basis. By the 15thof the month, all tenant events must be entered into AHFA DMS. Tenant Events must match the information in the household file selected for inspection. QAP POINTS TO REMEMBER It is VERY IMPORTANT to check the tenant data entered in AHFA DMS for accuracy. If Owner or Management Company acquires a Project and discovers issues such as excessive file corrections, building or unit repairs, it must submit a report to AHFA within 60 days of taking possession of the Project. Include sufficient details of the repairs to be completed Include a deadline to complete the repairs

  4. QAP POINTS TO REMEMBER It is VERY IMPORTANT to send correction to AHFA before the deadline If corrections are received after the deadline the following will happen: Point deductions can double For any finding listed on the Non-compliance letter, will now be assessed a 2-point deduction

  5. QAP AHFA ONLINE DMS All Tenant Data must be entered by the 15thof each month for the prior month. Same Household Same Move-in Date Same Move-in Income Recertification (dated within 120 days before the Effective Date The tenant data in AHFA ONLINE DMS must match the following household file information: By the 1stbusiness day in February, all tenant events from January 1st through December 31stof the previous year must be entered and finalized in AHFA ONLINE DMS.

  6. Once a project is acquired and issues are discovered such as Extensive File Corrections Tenant Data Corrections Building or Unit Repairs QAP PROJECT ACQUISITION The owner/management must submit a report to AHFA within 60 days of taking possession of the Project. The report must include Details of the Repairs to be completed Completion Deadline

  7. QAP UNIT HEALTH & SAFETY ONE POINT PER OCCURRENCE Missing fire canisters above the cooktop surface or temperature limiting plates on the cooktop surface Exposed electrical wiring or electrical hazards in tenant accessible areas Insect infestation Failure to inform AHFA staff in advance of any units currently infested and/or being treated for bed bugs or similar infestation Missing, non- charged or empty fire extinguishers Missing or non- working smoke detectors If temperature limiting plates are used make sure to have supportive documentation

  8. QAP UNIT HEALTH & SAFETY ONE POINT PER OCCURRENCE Missing, broken, or loose handrails or steps (no buffer)

  9. EXPOSED ELECTRICAL WIRING OR ELECTRICAL HAZARDS IN TENANT ACCESSIBLE AREAS QAP PROJECT & COMMON AREA ONE POINT PER OCCURRENCE TRIPPING HAZARDS (SIDEWALKS, PARKING LOTS, OR OTHER ACCESSIBLE AREAS) MISSING, BROKEN OR LOOSE HANDRAILS OR STEPS BOARDED, BROKEN OR MISSING WINDOWS MISSING OR DAMAGED ENTRY/EXIST DOORS OR BATHROOM DOORS

  10. QAP PROJECT DEFICIENCIES 1 POINT PER OCCURRENCE A project amenity listed on the project application which is found to be missing or damaged without evidence of immediate notification to AHFA

  11. QAP UNIT DEFICIENCIES 1 POINT PER OCCURRENCE A unit amenity listed in the project application which is found to be missing or damaged Units that have been vacant more than 30 days and are not immediately available for occupancy. More than 25% of the total units inspected Removal of previous household s items Repairs completed to the walls and floors Cleaned carpets Cleaned walls General maintenance completed

  12. QAP UNIT DEFICIENCIES 1 POINT PER OCCURRENCE Noncompliance issues, that have not incurred an automatic point deduction, found in more than 25% of inspected units.

  13. QAP OTHER GENERAL DEFICIENCIES 2 points per occurrence will be assessed for other general deficiencies cited as a finding and were not corrected by the due date
