Why do we need to change Why do we need to change? Historic approach to health inequalities has not delivered priorities decided by NHS/LA, one way to deliver services across Wirral etc Health service focus on treating illness Inequalities and unexplained variances in health outcomes continue Impact of pandemic and cost of living crisis increasing health inequalities Levels of deprivation increasing Joint working in pandemic has shown real benefits need to connect more with our communities Neighbourhood is geographic boundary (postcodes) PCN footprints not changing
Improving Wirral residents health through a community led approach Our Model = Neighbourhoods Model will form the foundation for how we on Wirral and our health and care system will tackle health inequalities and improve well being It will be led by community leaders - bottom up approach to tackling health inequalities and including the wider determinants of health. Population health and local intelligence joined focus on deep local insight Each Neighbourhood will decide their particular priority to focus on community participation Relationships in the neighbourhood are key Work will be prevention focused wider determinants and clinical approach Understand the neighbourhood to ensure there is a community asset approach
Our vision for our Neighbourhoods Our vision for our Neighbourhoods Connected A community who connect with each other, VCFSE groups (voluntary, community, faith and social enterprise) and statutory services. Increased social and emotional wellbeing opportunities to form meaningful relationships and reduce social isolation. Accessible services and activities that promote equal opportunities across our diverse community and allow for residents voices to be heard and understood. Confident A community with the resource and confidence to do rather than feeling done to utilising the fantastic work that neighbourhoods are already doing and supporting innovative approaches to reach the whole community. Building capacity within neighbourhoods to create sustainable engagement that improves wellbeing and drives health outcomes. Control A community who are empowered to be in control of the decisions that are made, ensuring that the process is meaningful and open. Promoting increased community engagement and motivation for all residents to get involved . Reducing statutory control and tokenistic feedback that allows community leaders and residents to take ownership of their neighbourhoods.
Our Principles Transparent - Honesty about what we are trying to achieve with the neighbourhood model, the scope of the model (what we can do and what we can t), open about capacity of services & waiting times, clarity about the process and managing expectations of change through an ambitious but realistic approach. Co-Production Community participation is essential to lead the change, utilising local people s strengths to plan, deliver and review the approach and methods of tackling inequalities within each individual neighbourhood. Ensuring that there is equality in decision making and shared accountability between community leaders, residents, the NHS and Local Authority. Insights & Data Recognising the importance of local insight that community leaders and residents provide whilst utilising the population health data outlined by statutory services to allow neighbourhoods to set their own individual priorities. There will also be a partnership between with statutory health and care organisations, enabling deep insight into effective service delivery. Prevention Switching our focus to preventing ill health through the promotion of wellbeing alongside treatment. Utilising an asset-based approach and building on what already works well to improve community wellbeing. Strategic Approach adopt the Wirral Health and Care Plan principles
Scope What we can do The aim is to improve health outcomes via a neighbourhood approach. The focus in each neighbourhood will be identified from local intelligence and population health data. Our focus will be broader than health and care and acknowledge the need to address wider determinants of health. We will test out new ways of working/approaches to delivering services. We will deliver within any allocated funding available to support projects. We will explore options to co-locate services in the geographic area of the neighbourhood or explore ways of increasing access to services, and target resources to where they will have the greatest effect on people in the neighbourhood. It is important that we focus on prevention of ill health and promotion of good health, both physical and mental. What can we can t do? We can t propose changes to health and care services that may potentially cause harm to people or are not legal. We can t propose changes to health and care services that are not sustainable, such as there is not the work force to support, or financially viable.
Progress to date Steering Group membership agreed and meeting monthly 4 VCFSE reps Principles, vision and scope developing Phased approach Identify two neighbourhoods communities trail blazers process now underway Agree Core group membership (two trail blazers) and start to meet monthly support from Place management team Engagement plan for the two neighbourhoods variety of ways including an engagement event. Population health dashboards being developed for each including activity eg ED attendance etc. Key issues Relationships are key and the bottom up approach must allow time for this Get champions shiny stars and GP champions Agreement to a relationship document for each core group as we progress and measure maturity Name the neighbourhoods agreeing how! Agree priority/focus of work in neighbourhood and outcomes to be collected
