  2. What makes a voter registration application incomplete? Applicant s name. Residence address (or map, description, or diagram showing location ). Mailing address (other than zip code), unless applicant provides residence address which receives USPS mail delivery for applicant. Date of Birth Voter Identification Number or indicate none on their registration form.

  3. What makes a voter registration application incomplete? Applicant s answer to US citizenship question. Applicant s answer to age question: Will applicant be at least 18 years of age on or before election day? Map or description to locate voter residence if residence address has no street number or name on application.

  4. What makes a voter registration application incomplete? Applicant s signature, which indicates that: Applicant meets requirements for approval; All information and statements on application are true; Applicant understands that if applicant signs statement, knowing that it is not true, the applicant is committing the crime of perjury.

  5. What DOES NOT make a voter registration application incomplete? Applicant s failure to indicate if registration is new or address or name change to existing record. Applicant s failure to write SAME on mailing address line if residence address receives USPS mail. Applicant s failure to indicate county of residence. Applicant s failure to indicate gender. Applicant s telephone number or email address.

  6. County VR Office Procedures for Incomplete voter registration application (IC 3-7-34) Promptly make 1 effort to contact applicant by mail AND 1 effort to contact applicant by telephone if number provided. OK to document applicant s responses by telephone EXCEPT for age and citizenship questions or signature. To complete age and citizenship questions, applicant must submit sworn statement under penalties of perjury to answer (most often, a new VR application). For missing signature, submit new VR application.

  7. County VR Office Procedures for Incomplete voter registration application (IC 3-7-34) DEADLINES: Applicant has until Day 29 before the election to provide answer to age and citizenship questions. Applicant has until Day 10 before the election to provide answer to other incomplete information (including signature)

  8. County VR Office Procedures for Incomplete voter registration application (IC 3-7-34) NEXT STEPS: If application remains incomplete on Day 10 before election day, the voter registration office certifies list of incomplete applications to county election board; and Rejects incomplete application.

  9. County VR Office Procedures for Incomplete voter registration application (IC 3-7-34) NEXT STEPS (Verification of DLN/SSN numbers): If application includes Indiana Driver s License Number or last 4 digits of Social Security number, and BMV unable to verify numbers on application by Day 10 before election day, the voter registration office includes this application in list of incompletes to county election board and rejects incomplete applications.

  10. County VR Office Procedures for Incomplete voter registration application (IC 3-7-34) NEXT STEPS (Certified Statement of Acceptance): If person who took custody of VR application does not complete Certified Statement of Acceptance (but application otherwise complete), county VR office: Approve application if otherwise complete and eligible. Make efforts to contact custodian of form to complete Certificate. Within 3 days after receipt, notify county election board of possible election law violation for action by CEB under IC 3-6-5-31. This only applies to state voter registration forms, not federal.
