The Case Plan The use of a case plan for case management is one aspect of best practices in supervision. A case plan documents and organizes the work needed for the client to move towards pro-social behavior and/or recovery.
The Case Plan (CP) The CP uses the LSI-R s criminogenic needs. Completing the CP is best done in collaboration with the client to plan out Goals, Objectives, and Tasks associated with the criminogenic needs. The CP also contains the Stages of Change that the officer estimates the client is in with regards to the criminogenic needs. Stages of Change is taught in the Motivational Interviewing curriculum.
Case Plan in CYZAP After a trial run on updating and modifying the old case plan, a new case plan in the CYZAP system became live in August 2020. The use of CP is taught in the Collaborative Case Work training. At April of 2022, there are over 500 case plans in the system. This is a presentation on the data about the Stages of Change associated with the criminogenic needs as shown on the case plans.
